In this study the mechanism of sinus bradycardia has been studied in 27 patients with epidemic hemorrhagic fever (EHF) by clinic electrophysiologic examination and the pharmacological provocation test. Of 27 patients, 18 were complicated with sinus bradycardia (66.7%). The diagnosis of sinus dysfuncation was made in five of them by abnormal electrophysiologic manifestation and the pharmacological provocation test. In the other 13 patients the electrophysiologic parameters were in the normal range though significantly higher than those in the control group and the non-bradycardia group (p<0.05).

本文应用临床电生理检查及药物激发试验,探讨了流行性出血热(EHF)并发心动过缓的机理,27例EHF中并发心动过缓18例,占66.7%.其中5例窦房结功能经电生理及药物试验揭示明显异常,符合窦房结功能不全诊断标准.余13例电生理参数较非心动过缓组及对照组明显增高,但尚在正常范围.提示EHF并发心动过缓病例中部分系窦房结功能不全所致,推测可能与窦房结区充血、水肿、炎症、出血有关;部分病例则可能是植物神经功能失调,迷走神经张力增高所致.

It is concluded that pre—implantation electrophysiolog ical studies are useful for the decision of the application of pacemaker and the selection of propper pacing mode.

对安装起搏器前作过临床电生理检查106例病人的资料进行分析,其中房室传导阻滞与病窦各为53例。全部病例均作了希氏束电图,病窦患者加作窦房结功能检查及房室交界区起搏与传导功能检查。房室传导阻滞病例的阻滞部位在房室结者占30.2%,在结下者占64.2%;53例中40例为完全性阻滞,其中9例还合并另一部位阻滞。病窦患者53例中大多数的窦房结功能测定结果异常,部份病例的房室结起搏功能和房室传导功能也有障碍。表明:在安装起搏器前进行电生理检查可以准确判定房空阻滞的具体部位和多部位阻滞,揭示病窦患者合并的房室系统起搏与传导功能的异常,这对于决定是否安装起搏器及选择起搏器的类型均有指导意义。

The etiology and electrophysiology in 200 patients with implantation of the pacemaker were ana- lyzed. The results showed that 69 patients with sick sinus syndrome and 131 with atrioventricular block were treated with pacemaker, and that coronary heart disease was the major cause of pacemaker implantation. The results showed that 69 patients with sick sinus syndrome and 131 with atrioventricular block were treated with pacemaker, and that coronary heart disease was the major cause of pacemaker implantation. Ninety-four cases of the 200 patients with implantation of the pacemaker underwent electro-physiological evaluations before the implantation. It was suggested that intra-and intra-his bundle block were important causes of pacemaker implantation in patients with atrioventricular block. The electro-physiologic study was important for patients with bradycardia before pacemaker implantation.

对200例首次安装心脏起搏器的患者进行了病因学和临床电生理学研究。结果提示,冠心病是导致过缓性心律失常而安装起搏器最主要的原因,安装起搏器的房室传导阻滞患者大多是希氏束内和希氏束下阻滞。对过缓性心律失常患者安装起搏器之前进行临床电生理检查有重要的实际价值。
