Objective Development and investigation of a new artificial ligament made of polyester woven and nonwoven felt and to evaluate its biocompatibility and biomechanical behavior in the animal implantation model. Methods Surgical resection and replacement of a 3 cm long posterior tibial tendon in both left and right hind limbs with artificial ligament were performed in 16 adult healthy sheeps.The animals(n=16)were divided equally into 4 implantation groups and each of the 4 animals was sacrificed at an interval of 3,6,9 and 12 months after opera- tion.Microscopic,TEM histopathologic examinations and biomechanical tensile strength tests were performed to evaluate the efficacy of the artificial substitutes.The behavior of the bone-ligament fixation was also evaluated in an additional sheep by performing bone tunnels and investigating the implantation of the artificial ligament.Results 16 animals survived after operation without evidence of infection.Comparable degrees of collagen fibers with pres- ence of reticular fibers and ingrowth of small blood vessels were present in the 3 months implants.During the same period,active fibroblasts with ingrown of island-like collagen tissues were also evident.After 6 months of implan- tation,the island-like collagen tissues were all well connected.Bands of well-ordered collagen fibers were seen in the vicinity of the polyester fibers.9 months after operation,there was a progressive increase of dense and abun- dant collagenous fibrous material and the collagen tissue surrounding the polyester fibers.12 months after opera- tion,full thickness ingrowth of collagen tissue including the interior interstices of polyester woven structure was observed.The collagen fibers had a more definite orientation,with majority of the fibers parallel to the long axis of the ligament implant.The tensile strength of the artificial ligament was tested before and after the operation.The control group ruptured at(1 334±43.21)N.The implants had been shown to have an average tensile failure strength of(1 291±49.11)N at 3 months,(1 239±109.33)N at 6 months,(1 188±95.45)N at 9 months and (1 379±240.30)N at 12 months after implantation.Compared with the control group,the tensile strength of the ligament implants had no significant biomechanical deterioration after 12 months of implantation(P>0.05).The fixation of the bone-ligament interface was satisfactory.A tight binding collagen tissue with ingrowth of chondro- blasts was observed after 3 months of bone tunnel implantation.Conclusion The new artificial ligament made of polyester woven and nonwoven felt is a biocompatible and biomechanical retainable prosthesis for ligament substitu- tion.It appears to be very promising for clinical application.
