The flaps can be rotated primarly in all cases, (If there are a lot of fats under the skin, the flap necrosis may occur, which, therefore, Should be watched out for).

A technique aiming at protecting the "marginal branch" has been developed, thus the area of the skin flap is expan- ded. As two heads of the sternocleidomastoid muscle can be separated, one head of the muscle is used safely in repairing large defects in the maxillofacial region, and the operation has been successful in 4 cases.This method not only maintains the function of clavicular head of the muscle, preventing the production of postoperative wryneck, but also allows the use of the innervated sternal head of the muscle to repair the loss of the masseter muscle.

本文通过对25具(50侧)成人尸体所进行的解剖学观察与测量,从肌皮动脉“缘支”同样也是肌皮瓣血供主要来源这一新的观点出发,在手术设计上将肌皮瓣的肌蒂改为复合组织蒂,并提出了保护“缘支”的手术操作方法,从而扩大了皮瓣的应用面积。根据胸锁乳突肌两个头的可分离性,手术中安全地切取了单头胸锁乳突肌皮瓣,并用以修复颌面部大块组织缺损获得成功,这不仅保留了胸锁乳突肌锁骨头的功能,防止术后出现“歪颈”,而且有的病例还用胸骨头代替嚼肌,通过保留其运动神经,使已缺损的嚼肌生理功能得到了恢复。

Eight rats weighing between 150 and 300g were used. The central location of the sternornastoid nerve (SN) and trapezius nerve (TN) of rat were investigated using the retrograde HRP method. Diameters of the labelled cell bodies were measured by an image analyser.After application of HRP crystals to the cut SN or TN, labelled cells of the SN were seen in the posteromedial and lateral nuclear groups of the ipsilateral anterior horn extending from the deoussation of pyramids (DP) to 04 segments. Location of the HRP labelled cells for the TN were found mainly in the lateral and anterior nuclear groups ipsifaterally from 01 to 06 segments. In comparison to TN, the HRP-positive cells of the SN were localized more rostrally and medially.The labelled cells of the SN were observed in three animals by image analyser (VIDS III). Mean area and sphere diameters of the labelled cell bodies measured 407.68μm2 and 22.25μm respectively. The sphere diameters measured in an image analyser are 49 % of micrometer measurements.

用HRP逆行追踪法和图象分析研究了大鼠副神经的分支:胸锁乳突肌神经和斜方肌神经的中枢定位。标记细胞位于副神经脊髓核内,自同侧延髓锥体交叉尾侧至脊髓C_6节段。标记的胸锁乳突肌神经运动神经元偏向颅侧,斜方肌神经运动神经元偏向尾侧,两者的中枢定位略有不同,且有重叠现象。还对3只大鼠标记的胸锁乳突肌神经运动神经元进行图象分析测量。
