In order to explore the value of sternocleidomastoid myoperiosteal flap for laryngotracheal reconstruction, eight dogs were included in the experimental study. After resecting the anterior body of the cricoid cartilage and tracheal tissue, the sternocleidomastoid myoperiosteal flap was used to close the defect immediately. In order to explore the value of sternocleidomastoid myoperiosteal flap for laryngotracheal reconstruction, eight dogs were included in the experimental study. After resecting the anterior body of the cricoid cartilage and tracheal tissue, the sternocleidomastoid myoperiosteal flap was used to close the defect immediately. All dogs were killed at two、four、six and eight months after operation. It was found that the sternocleidomastoid myoperiosteal flap had incorporated completely into the defect. Histologic studies by light and scanning electron microscopies showed complete epithelialization with no granuloma formation on surface of the flap and apparent metaplastic new bone formed in the periosteum which was crucial to the stability of the air way and long term success of the procedure. Five patients who underwent subglottic and tracheal reconstrction with sternocleidomastoid myoperiosteal flap were successfully decannuated and had normal exercise tolerance. The results indicate that the sternocleidomastoid myoperiosteal flap is an ideal transplant for laryngotracheal reconstruction.

为探讨胸锁乳突肌肌骨膜瓣在喉气管重建术中的作用,切除狗环状软骨弓及部分气管前壁造成缺损,取胸锁乳突肌肌骨膜瓣修复,分别于术后2个月~8个月处死动物,观察修复情况,并取材作光镜及扫描电镜检查。结果发现:胸锁乳突肌肌骨膜瓣行喉气管重建具有良好支撑力,修复的呼吸道通畅,无瘢痕及肉芽组织形成,移植骨膜表面为粘膜上皮覆盖,深层有新骨组织形成。临床治疗5例患者,均拔除气管套管,恢复正常呼吸功能。胸锁乳突肌肌骨膜瓣是理想的喉气管重建材料。

Four months after the operation, the post cricoarytenoid muscles were sampled for histologic and morphologic investigation. The results showed that all the experimental animals from the two groups had successfully regained abduction function. 将实验犬左侧喉返神经及喉上神经外侧支切断,造成单侧联合性声带麻痹动物模型.分别选用同侧颈袢胸骨舌骨肌支肌蒂及副神经胸锁乳突肌支肌蒂移植于该侧环杓后肌.手术前后进行录音、喉镜检查及声带录像,肌电图检查,术后4个月取左环杓后肌标本进行形态学和组织学检查.结果显示两组实验动物均成功地恢复了麻痹声带的外展功能.文章对神经肌蒂环杓后肌移植的手术术式进行了探讨.

Objective:To compare the results and effect of different neuromuscular pedicle (NMP) embeded in posterior cricoarytenoid muscle.Method:In 22 dogs,combined vocal cord paralysis models were made. The first group (10 dogs) were embeded by sternohyoid muscle pedicle of ansa cervicalis, the second (10 dogs), accessory sternocleidomastoid neuromuscular pedicle, the third (2 dogs), control group. Result:Evaluation was possible in 19 dogs. Reinnervation of the left posterior cricoarytenoid muscle occurred in 17 dogs...

The first group (10 dogs) were embeded by sternohyoid muscle pedicle of ansa cervicalis, the second (10 dogs), accessory sternocleidomastoid neuromuscular pedicle, the third (2 dogs), control group. Result:Evaluation was possible in 19 dogs. Reinnervation of the left posterior cricoarytenoid muscle occurred in 17 dogs following NMP. Return of spontaneous abduction in the operated side with NMP was noted after 4 months postoperatively. There was little difference between the two NMP groups.Conclusion:NMP was successful to reinnervate the posterior cricoarytenoid muscle.

目的:比较不同神经肌蒂环杓后肌移植术治疗声带麻痹的疗效,为临床选择术式提供参考。方法:建立联合性喉麻痹模型,分别用颈袢胸骨舌骨肌肌蒂(简称颈袢组)和副神经胸锁乳突肌肌蒂(简称副神经组)移植于环杓后肌,以恢复声带外展功能。术后4 个月,分别进行录音、直达喉镜和电视喉动态镜观察声带运动并录像,喉肌电图检查,环杓后肌透射电子显微镜检查和酶组织化学检查并进行电脑图像定量分析。结果:术后颈袢组和副神经组犬吠声均恢复正常,浅麻醉下直达喉镜和动态喉镜检查见双侧声带运动对称,喉肌电图检查均可从环杓后肌上记录到与呼吸同步的动作电位。组织化学及电子显微镜检查两组骨骼肌横纹清晰,神经纤维芽支再生明显,且已与环杓后肌建立了较多的运动终板,肌糖原正常。经统计学处理,两组间无显著性差异( P > 0.05)。对照组肌纤维明显萎缩,肌纤维中几乎无运动终板。结论:神经肌蒂移植术是一种治疗声带麻痹切实有效的方法,颈袢组与副神经组两种术式均能使麻痹喉肌获得有效神经再支配,为临床术式的选择提供实验依据
