Methods: The morphological types of peroneus brevis tendon and the sizes of tibia and fibula were observed and measured on 30 sides of adult lower limb specimens, and two specimens were tested according to the operational need. The anteroposterior diameter of fibula was 2.1 ± 0.5 cm; the anteroposterior diameter of tibia was 3.4 ± 0.4 cm; the distance between the anterior margins of tibia and fibula was -2.8±0.4 cm, 1.3 ± 0.3 cm between the posterior margins of tibia and fibula and 5.3 ± 0.4 cm between the medial margin of tibia and lateral margin of fibula. Conlusion: Peroneus brevis tendon diverted partly to repair the separation of tibiofibular syndesmosis is feasible.

目的:为部分腓骨短肌腱转位修复下胫腓联合分离术提供解剖学基础。方法:30侧成人下肢标本,对腓骨短肌腱进行形态学观测。结果:在踝关节平面上1cm处,腓骨短肌腱上长(12.5±2.2)cm,下长(9.5±1.0)cm,腓骨前后径为(2.1±0.5)cm,胫骨前后径为(3.4±0.4)cm,胫、腓骨前缘之间距离为(2.8±0.4)cm,腓骨短肌腱距腓骨外缘(1.3±0.3)cm,胫骨内缘距离腓骨外缘(5.3±0.4)cm。结论:部分腓骨短肌腱转位修复下胫腓联合分离具有可行性。

Objective: To provide further anatomical basis for harvest technique of vascularized fibula shaft. Methods: Twenty four formalinized adult cadaver calves were used in this investigation. The fibula was divided into four segments averagely. Observation was made for the point of fibular attachment, nervimuscular branches of relative muscles and the characteristics of the origin of fibular nutrient vessels. Objective: To provide further anatomical basis for harvest technique of vascularized fibula shaft. Methods: Twenty four formalinized adult cadaver calves were used in this investigation. The fibula was divided into four segments averagely. Observation was made for the point of fibular attachment, nervimuscular branches of relative muscles and the characteristics of the origin of fibular nutrient vessels. Results: The average width of the fibular surfaces decreased gradually from peroneal surface, flexor surface, tibialis posterior surface to extensor surface in order. The origin of the nervimuscular branch of peroneus brevis was 99.2±27.0 mm distal to the fibular head. The branch ran most closely to the peroneal surface in the two fourth segment and proximal half of the three fourth segment. The first nervimuscular branch of extensor hallucis longus was 100.7±18.7 mm distally to the fibular head. It ran most closely to the fibular anterior border in the two fourth segment. First, second and third arteria arcuate and arteria nutricia peroneus concentrated on the two fourth segment of the fibula. Conclusions: The major "muscular cuffs" in vascularized fibula shaft harvest are extensor hallucis longus and tibialis posterior. During the harvesting, the nervimuscular branch of peroneus brevis can be easily injured in the two fourth segment and proximal half of the three fourth segment of the peroneal surface, and the nervimuscular branch of extensor hallucis longus can be easily injured in the two fourth segment of the fibular anterior border. The harvested shaft could contain the nutrient vessels most possibly when the proximal osteotomy level is through the middle point of the one fourth segment.

目的:为临床带血管腓骨干切取补充解剖学基础。方法:在24侧防腐固定的成年小腿标本上,将腓骨全长平均分为4段,解剖观测腓骨肌肉的附着及部分肌肉的神经支配情况,观测腓骨营养血管的起始位置的特征。结果:腓骨干上附着肌肉的宽度从大到小依次为腓骨肌面、屈肌面、胫骨后肌面和伸肌面。腓骨短肌的肌支发出处距腓骨头尖(9.9±2.7)cm,于腓骨中2/4段和中3/4段的上半部分最贴近腓骨骨面,长伸肌的第1肌支起始位置距腓骨头尖(10.1±1.9)cm,于腓骨的中2/4段紧贴腓骨前缘,腓骨滋养动脉和第1 ̄3弓状动脉集中于腓骨中2/4段。结论:(1)带血管腓骨干所可能切取的“肌袖”主要为长屈肌和胫骨后肌;(2)腓骨肌面的中2/4段和中3/4段的上半部分为手术时容易损伤腓骨短肌肌支的部位;(3)腓骨中2/4段近腓骨前缘是长伸肌神经支最易损伤的部位;(4)近端截骨点位于腓骨上1/4段中点的切取可以最大程度地包含腓骨的营养血管。
