Objective To investigate the protective effect of lidocaine on motoneuron and dorsal root ganglion sensory neuron and provide the basis for clinical use. Methods Fifty-two SD female adult rats were divided into control group and lidocaine group randomly, with 26 each. Transverse incision was done infraclaviclely, and the brachial plexus at two sides were exposed. In the lidocaine group, 1% lidocaine (2μl) was injected into C_7 nerve root 5 mm distal to intervertebral foramen under epineurium. In control group 2μl normal saline was injected. Vetrograde using True Blue neuron tracing and neuronal specfic enolase (NSE) immunohistochemistry, the number of anterior horn motoneuron and dorsal root ganglion sensory neuron of cervical 7 roots were counted and the marked neuron activity was detected. Results The number of dorsal root ganglion sensory neuron decreased obviously in control group during postoperative 4 to 16 weeks, and after postoperative 8 weeks in the lidocaine group. The number of sensory neuron in lidocaine group was much larger than that in the control group. The number of motoneuron in the ventral horn in the spinal cord in 2 groups decreased significantly on postoperative 8 weeks. But the number in the lidocaine group was larger than that in the control group. Sensory neuron and motoneuron traced by True Blue in 2 groups could all be double dyed by NSE immunofluroscene technique. Conclusion Epineurial injection of lidocaine before transection of nerve root can significantly prevent sensory and motoneuron from retrograde degeneration, directly at the early period and indirectly at the late stage.

目的 研究利多卡因对神经元的保护作用 ,为临床应用提供实验依据。方法 SD雌性大鼠 ,利多卡因组 2 6只 ,对照组 2 6只。锁骨下横切口 ,暴露双侧臂丛神经。利多卡因组于颈7神经根距椎间孔以远 5mm处在外膜下注射 1%利多卡因 2 μl ,对照组则注射 0 .9%生理盐水 2 μl。利用TrueBlue逆行示踪技术和NSE免疫荧光双标技术 ,精确计数神经元和检验标记神经元的活性。结果 背根神经节感觉神经元计数 :对照组神经元数量在术后 4~ 16周呈持续减少 ;利多卡因组在术后 8周才明显减少。同一时间点利多卡因组的数量明显多于对照组 (P <0 0 5 )。脊髓前角运动神经元计数 :术后 8周 ,利多卡因组和对照组的神经元数量均明显减少 ,而利多卡因组的运动神经元数量明显多于对照组 (P <0 0 5 )。各组中 ,TrueBlue标记神经元全部被NSE免疫荧光双标。结论 神经根切断前外膜下注射利多卡因对脊髓前角运动神经元和背根神经节感觉神经元都有显著的保护作用。神经元逆行性退变可能存在双重诱发机制。
