The Picture of bile Plugs and intrahePatoc-ytic bile Pigments, which appeared stalactite-like under SEM, was described. A Process was reconstructed to define how the intrahepatocytic cholestasis leads to chole-coagulative necrosis and the static bile Pigment clot is eventually surrounded by collagen fibers. The electron microscopy also showed that the regeneration of remnant hepatocyte islets was bud-like, with the newly formed Pseudoducts not communicating with the original biliary duct system, and thet the regeneration of hepatocyte islets was restricted by the hyperpfoliferated collagen fibers around them.

Clinical chemistry parameters (hepatic function, glucose metabolism and protein metabolism) were measured preoperatively and on day 1 and day 6 postoperatively. Result: (1)Total bilirubin, ALT, AST on day 6 postoperatively decreased in Group Ⅰ and Group Ⅲ (P <0. 05), but increased in Group Ⅰ (P < 0.05 ). (2)The blood glucose and insulin levels were normal in Group Ⅱ and Group Ⅲ, but increased in Group Ⅰ(P < 0.05). (3)Serum albumin, transferrin and prealbumin levels increased in group Ⅱ (P < 0. 05) butdecreased in Group Ⅰ and Group Ⅲ (P<0. 05). Conclusion: (1 )TPN with fat emulsion could improve hepatic function, glucose and protein metabolism of surgical patients with liver disease. (2)The dosage of 1g·kg-1·d-1 of fat ernulsion was preferable to 2g· kg-1·d-1.

将32例肝脏外科疾病患者随机分为Ⅰ组(单能源TPN组10例);Ⅱ组(双能源TPN组11例,其中脂肪乳剂用量为1g·kg-1·d-1);Ⅲ组(双能,TPN组11例,其中脂肪乳剂用量为2g·kg-1·d-1)。术后按组别给予TPN支持共6天,术前1天、术后第1和第6天测定肝功,糖代谢及蛋白质合成代谢指标。结果:①Ⅱ、Ⅲ组术后第6天肝脏酶学指标明显下降(P<0.05),而Ⅰ组仍高于术前水平(P<O.05);②Ⅱ、Ⅲ组术后糖代谢基本恢复正常,而Ⅰ组出现高血糖症及高胰岛素血症(P<0.05);③Ⅱ组肝脏蛋白质合成水平恢复术前水平或略有提高(P<0.05),而Ⅰ和Ⅲ组术后蛋白质合成功能仍低(P<0.05)。结果提示:含脂肪乳剂的TPN支持对肝脏外科患者术后的肝功恢复有益,能促进蛋白质合成及肝细胞再生,并且在进行TPN支持时按1g·kg-1·d-1给予脂肪乳剂较为安全合理。

Objective To confirm the effect of auxiliary partial heterotopic liver transplantation(APHLT)in the treatment of fulminant hepatic failure (FHF). Method On 30 rat models of FHF,62% of partial liver grafts were implanted below the host residual liver with 28 successful operations. The survival rate, biochemical test for liver function, 99m TcHIDA liver scintigraphy and pathological changes of the grafts and host livers were studied. Result The survival rate in 48 hours was 714% compared with 0% in 15 FHF animals. 14 days after the operation,the host liver regenerated obviously and the liver function recovered while the graft shrank gradually. 30 days later, the graft became fibrosis completely. Conclusion Depending on the temporary function support of the graft, the native residual liver could regenerate and restore its function while the graft tended to atrophy.

目的探讨辅助性肝移植对急性肝功能衰竭(简称肝衰)的治疗作用。方法在30只肝功能衰竭大鼠模型上,于原残肝下移植同系异体62%部分肝脏,手术成功28例。观察了肝衰组和移植组大鼠存活、血液生化、99mTcHIDA肝显像、残肝和移植肝组织细胞形态改变。结果肝衰组(15只)和移植组(28只)大鼠48小时存活率分别为0%和71.4%。移植术后14天残肝细胞明显增生,肝功能基本恢复正常,辅助肝开始萎缩,术后30天辅助肝完全纤维化。结论辅助性部分肝脏异位移植可为衰竭之残肝提供暂时性功能支持。原残肝细胞再生,功能恢复过程中辅助肝逐渐萎缩、废用
