Objective: To evaluate anatomic changes after upper lobectomy. The atelectasis like shadow was particularly studied.Materials and Methods:We compared the anatomic changes of X ray with CT scan which had performed at the same time in 42 lung cancer patients with right or left upper lobectomy.Results:We suggested that these anatomic changes were due to the postoperative vacuum cavity. The compensative balance mechanisms were: (1) Diaphragm raised (n=42); (2) The organs of mediastinum, such as bronchus and heart shifted (n=40); (3) Mediastinum adipose tissue displacement (n=9); (4) Occurred pleural thickening after effusion (n=16). In patients with mediastinum adipose tissue shift the main presentation in the X ray films was: there was a wedge shadow with unopaque density of cor diaphragm angle, some of them had a large range of shadow, and seemed atelectasis, but CT could proved they were adipose tissue.Conclusion:X ray and CT scans possesse advantages for itself in observing the anatomic changes after lobectomy, we emphasized that the density of shadows may help us to give a correct explanation.

目的:对上肺叶切除术后改变,重点在肺不张样阴影的评估。材料与方法:搜集原发肺癌经手术切除左/右肺上叶后正位胸片42例与同期CT片对比研究。结果:认为上肺叶切除术后的解剖变化是由于术后空腔形成的真空状态引起的,其代偿平衡机制为:(1)膈肌升高(42例);(2)纵隔器官如气管及心脏移位(40例);(3)纵隔脂肪组织移位(9例);(4)胸腔积液和随后形成的胸膜肥厚(16例)。纵隔脂肪组织移位主要X表现为心膈角处边界清晰的楔状阴影,密度相对透明,范围较大者似肺不张,CT可证实为脂肪组织。结论:观察肺叶切除术后改变X线及CT各具优点,强调术后X线图像,有时只有借助密度测定才能得到解释

AIM To observe heart displacement and compression during off pump CABG and to analyze the hemodynamic changes in relation to the grafted arteries. METHODS 47 patients with coronary artery stenosis underwent off pump coronary artery bypass grafting, utilizing Octo puz Tissuetabiblization system (medtronic) and CTS Stabilization(cardiothoracic) to expose the anastomosic sites. RESULTS Some of the patients were given noradrenaline 0.02~0.20 μg·kg -1 ·min -1 in order to maintain MAP above 9.3 kPa in suturing right coronary artery (RCA), diagonal branch artery (D), and obtuz marginal branch artery (OM). During the surgery on the left anterior descending artery (LAD), stroke volume (SV) and cardiac index (CI) decresed 1 mL and 200 mL·min -1 ·m -2 ; RCA, SV and CI decresed 2 mL and 280 mL; D, SV and CI decresed 6 mL and 220 mL and OM, SV and CI decresed 1.6 mL and 220.0 mL·min -1 ·m -2 ( P >0.05). All the cardiovascular indexes returned to base line values 15 min after releasing the heart. CONCLUSION This study provides evidence that the hemodynamic changes associated with heart displacement and compression during off pump CABG are of short duration and within physiological limit.

目的 通过观察非体外循环下冠状动脉架桥手术时心脏的移位受压 ,分析与吻合冠状动脉相关的血液动力学改变 .方法  4 7例冠状动脉狭窄的患者 ,行非体外循环下冠状动脉架桥手术 ,采用 CTS和 octopuz两种固定器暴露吻合部位 .结果 少数患者在吻合右冠状动脉 (RCA)和对角支 (D) ;钝缘支 (OM)时需辅以去甲肾上腺素 0 .0 2~ 0 .2 0 μg·kg- 1·min- 1 以维持平均动脉压在 9.3k Pa以上 .在所有吻合区每搏量 SV和心脏指数 CI都有少许下降 ,前降支 L AD下降 1m L和 2 0 0 m L· m in- 1· m- 2 ,RCA下降 2 m L 和 2 80 m L·min- 1· m- 2 ,D下降 6 m L 和 2 2 0 m L· min- 1· m- 2 ,OM下降 1.6 m L和 2 2 0 .0 m L ,与吻合前相比无统计学差异 ,在心脏复位 15 min后 SV和 CI恢复原来水平 .结论 非体外冠脉架桥手术时 ,心脏的受压和移位可引起血液动力学的改变 ,以上改变是短暂的且在生理范围之内

Objective: To assess the patients' hemodynamics during off-pump coronary artery bypass graft (OPCABG) surgery. Methods: The heart rate(HR), mean arterial pressure (MAP), mean pulmonary arterial pressure (MPAP), mixed venous oxygen saturation (SvCh), central veneous pressure(CVP) and cardiac output (CO) were continuously monitored in 25 patients with complete OPCABG revascularization. Hemodynamic changes were recorded at the completion of the anastomosis before releasing coronary snaring and stabilization and compared to baseline(before anastomosis). Results: The mean age of the patients was (63. ±8. 5) years, and on average (2. 8 ±0.9) grafts per patient were performed. The MAP, CO and Sv02 significantly decreased in all territories (P < 0. 05 or P < 0. 01). The CVP significantly increased in all anastomosed distal blood vessels. The MPAP significantly decreased in RCA and CX territories ( P < 0. 05). The patients' hemodynamics was of no statistical significance except MAP in anastomosed all proximal blood vessels. Conclusion: Cardiac displacement and compression induced by exposure and anastomosis of distal blood vessels during OPCABG surgery bring significant decline in the patients' hemodynamics , especially in RCA and CX territories . These haemodynamic changes were transient and well tolerated under anesthetic technique.

目的:评估非体外循环冠状动脉旁路移植术(OPCABG)中血流动力学变化。方法:随机选择25例,在OPCABG期间连续监测心率(HR)、平均动脉压(MAP)、平均肺动脉压(MPAP)、混合静脉血氧饱和度(SvO_2)、中心静脉压(CVP)和心排血量(CO)的变化。记录心脏固定器释放前吻合各支血管期间的血流动力学变化以及血管吻合前(基础值)和全部吻合完毕后血流动力学变化。结果:患者平均年龄(63.4±8.5)岁,平均搭桥(2.8±0.9)支。吻合各支远端时,MAP、CO和 SvO_2与各自基础组比较显著性下降(P<0.05或P<0.01);CVP显著性上升(P<0.05或P<0.01);在吻合右冠状动脉(RCA)和左旋支(CX)期MPAP显著性下降(P<0.05);吻合近端时,除MAP外,其他参数的变化无统计学意义。结论:在OPCABG时,暴露和吻合远端血管,将引起心脏的移位和压迫致显著的血流动力学衰减,尤其见于CX和RCA.采取适当的措施,所有患者能很好地耐受这些血流动力学的变化。
