Objective: Through comprehensive treatment of the loose skin, muscle, lower eyelid septum and protruding orbital fat in the aged, this kind of operation can correct them and meantime correct the sunken deformity of semilunar curve on the area of oculonasal groove and infraorbital marginalis in some patients. Methods: A little improvement was made on the management of orbital septum and septal fat while doing the general lower eyelid blepharoplasty. Separate the plane between them. Orbicularis oculi and orbital septum to the infraorbital marginalis, incise the orbital septum transversely, then sewing up the orbital fat onto the periosteum of the infraorbital marginaslis in order to fill up and correct the sunken deformity of this area. If the orbital fat is excessive, it can also be excised. Results: In a total of 42 patients available for this kind of operation and followed up for 1~3 years from 1996 to 1999, we`ve obtained excellent cosmetic results without any complication. Conclusion: This method can achieve good results through correcting the protruding baggy deformity, skin wrinkles of the lower eyelid and sunken deformity of oculonasal groove area and infraorbital marginalis.

PURPOSE: To investigate the methods, merits and effects of temporal anterior hairline incision in internal fixation of zygomatic complex fracture (ZCF). METHODS: 99 cases of ZCF were treated via vertical incision in temporal anterior hairline. During dissection, the zygomatic branch and the frontal branch of the facial nerve, which crossed over the zygomatic arch, were protected. The deep fascia superficial to the zygomatic arch was horizontally incised, the periosteum was elevated at the fracture sites. Cases of type A1 were treated through the above approach; while minor periorbital and oral vestibule incisions were also used in cases of type B and C. The fracture segments were reduced accurately and fixed with titanium microplates. Coronal incision was not applied in all cases. RESULTS: The clinical results were good in 99 cases (86.0%). All the temporal anterior hairline incisions healed primarily. The rate of temporary facial paralysis, due to injury of the zygomatic and frontal branch of the facial nerve, was 24.3% ; but all recovered six months later, without any permanent paralysis. CONCLUSION: The temporal anterior hairline incision has many merits including clear field of operation , convenient manipulation, minimal trauma, less bleeding , high accuracy of reduction and inconspicuous scar.

目的:探讨颞部发际前缘切口在颧骨复合体骨折(ZCF)内固定术中的应用方法、优势和效果。方法:沿颞部发际前缘作纵行切开分离,注意保护跨越颧弓的面神经颧、额支,横向切开颧弓表面深筋膜和骨膜,暴露骨折部位。A1型病例仅采用颞部发际前缘切口,B、C型病例配合眶周及口腔前庭等小切口,利用该切口撬动整个颧骨体复位,重建颧骨体外形轮廓。精确复位后,采用微型钛板作坚强内固定。不作头皮冠状切口。结果:99例107侧ZCF患者术后疗效优良率达86.0%。颞部发际前缘切口均一期愈合。面神经颧、额支麻痹引起的暂时性瘫痪率为24.3%,6个月内全部恢复,无永久性面瘫发生。结论:颞部发际前缘切口具有术野清晰,操作方便,创伤小,出血少,提高复位固定准确性及瘢痕隐蔽等优点。

AIM: To observe and compare the impacts on nerve growth factor (NGF) expression in rats of three groups: microencapsulated rabbit sciatic nerve (SN) cells transplantations on rats with spinal cord injury (SCI), rabbit SN cells transplantations on rats with SCI and rats only with SCI groups.METHODS: The experiment was finished in the Centre of Medical Science Research, Jiujiang University from May 2003 to May 2005.①Preparations of different transplanted materials: The complete SN of 10 rabbits were used for making cell suspension; With the adjusted cell concentration of (2.0-2.5) ×106 L-1, the microencapsulated suspension was mixed with the alginate saline solution, and ejaculated to 20 mmol/L barium chloride saline solution by double-cavity ejaculator, then rinsed twice by saline. ②SCI rats models establishment: Totally 90 healthy SD rats were randomly divided into 3 groups with 30 rats in each: Microencapsulated rabbit SN cells suspension group (Microencapsulated group); Rabbit SN cells suspension group (Only suspension group); Only injured group. After anesthetized with 30 mg/kg peritoneal injection, T10 spinous process and vertebra lamina of rats were exposed as T10 center. The spinal cord tissues of 90 rats were removed from rats by spinal cord right hemisection. The gelatin sponges with the size of 2 mm×2 mm×2 mm were grafted as filling cage, and adsorpted 10 μL microencapsulated rabbit SN cells (Microencapsulated group) and 10 μL rabbit SN cells (Only suspension group) respectively; There was no graft in only injured group. ③NGF detection: Six rats were selected on each time point (1st, 3rd, 7th, 14th, 28th days) to take out the 2 cm spinal cord samples, and totally 18 sections of 6 samples in each group were detected with the immunohistochemistry staining. The plasm presented obvious brown-yellow meant the positive staining of NGF. The number of positive nerve cells in per square millimeter was counted.RESULTS: All the 90 rats were involved in the result analysis. ①The detection of NGF positive neuron: NGF expression was higher in the microencapsulated group than in only suspension group and only injured group on the 1st, 3rd, 7th, 14th, 28th days; On the 7th, 14th, 28th days, the expression of microencapsulated group increased significantly compared with other two groups 7 days: (28.55±2.26), (7.65±3.35), (19.35±2.39) /mm2;14 days: (30.52±3.51), (8.10±2.45), (20.45±3.45) /mm2;28 days: (27.50±4.50), (6.59±3.85), (17.50±4.65)/mm2,P < 0.01 or 0.05; On the 14th day, the NGF expression of microencapsulated group peaked (P < 0.01). ②Immunohistochemistry staining of NGF: Masses of brown-yellow particles were found in the microencapsulated group, most of them were distributed in the spinal cord anterior horn neurons and glial cells. The positive neuron particles were also found in the white matter and grey matter.CONCLUSION: The SCI recovery in rats can be significantly accelerated by transplanting microencapsulated rabbit SN cell suspension. Compared with only suspension group and only injured group, the NGF can express highly after microencapsulation, which can improve the repair and regeneration of injured spinal cord.
