Objective To investigate the feasibility of dynamic magnetic resonance diffusion tensor imaging(DTI) in rabbit models with implanted VX2 hepatic tumor grafts.Methods MRI and DTI images were obtained from 16 rabbit models with implanted VX2 hepatic tumor grafts(14,18,22,and 26 days after tumor implantation,respectively) and 4 normal rabbits.The average diffusion coefficient(ADC) and exponential ADC(eADC) were estimated and compared against pathological findings.Results The ADC values increased after tumor implantation but then decreased in the rabbit models,whereas eADC exhibited a pattern of reverse changes.These changes significantly differed from those in the control group.Coagulation necrosis and fibrous hyperplasia showed obvious increase as found by pathological examination.Conclusion Dynamic MR DTI quantitative analysis of rabbit models of implanted VX2 hepatic tumor can partially describe the growth behaviors of implanted liver cancer.

目的探讨对兔VX2种植型肝癌模型的成瘤过程进行动态磁共振张量成像(DTI)量化研究的可行性及价值。方法在肿瘤植入后第14、18、22、26天分别对4组共16只新西兰大白兔VX2种植型肝癌模型和4组共4只正常新西兰大白兔进行磁共振成像及DTI;动态量化分析平均弥散系数(ADC)、指数弥散系数(eADC)的变化规律并与病理结果对照。结果兔VX2种植型肝癌模型的ADC呈先递增后递减趋势;eADC呈先递减后递增趋势;与对照组间存在显著性差异;病理结果的多项分析中,瘤内凝固性坏死和纤维增生呈明显递增趋势。结论DTI的量化指标(ADC、eADC)能够动态观测兔VX2种植型肝癌模型的部分生长特性;为DTI对肝脏病变的临床应用提供一定的理论指导。

Objective:The changes both within lesions and normal-appearing white matter load on T2-weighted MRI in patients with ischemic leukoaraiosis(ILA) was investigated by diffusion tensor MRI(DTI) and the relation between the changes of DTI parameters and cognitive dysfunction was assessed.Methods: Thirty-four patiens with ischemic leukoaraiosis and 19 healthy volunteers underwent DTI,conventional MRI,and neuropsychological assessment. Results: In ILA group,on DTI,diffusity was increased within lesions and in normal-appearing white matter compared with the control group,FA was reduced.The changes of MD in the NAWM correlates with full scale intelligence quotient(IQ) and executive dysfunction,the changes of FA in the NAWM correlates with global cognitive impairment assessed by the MMSE. Conclusion: DTI can reveal pathological changes in normal-appearing white matter load on T2-weighted MRI in patients with ischemic leukoaraiosis,which correlates with cognitive dysfunctions.

目的:用核磁共振(MR I)弥散张量成像(DTI)研究缺血性脑白质损害(ILA)患者常规MR I T2加权像上脑白质损伤区和一些形似正常脑白质(NAWM)区的变化及其与认知功能之间的联系。方法:对34例ILA患者和19例健康对照者进行常规MR I、DTI及神经心理学检查。结果:DTI扫描时,ILA组的脑白质损伤区及NAWM区的平均弥散系数(MD)值较对照组NAWM区的MD值升高,部分各向异性指数(FA)值降低;ILA组NAWM区的MD值变化和韦氏全智商及执行功能障碍相关,FA值的变化和简易智能状态检查量表(MMSE)衡量的总体认知损害相关。结论:DTI可显示ILA患者常规MR I上NAWM区的病理变化,其变化与认知障碍相关。

Objective The functional dynamics within segregated cortical areas are believed to beconstrained by both intrinsic and extrinsic anatomical connections.The objective of this study was to investigatethe relationship between functional deficit of visual cortex and disruption in callosal and optic radiation devel-opment in human ametropic amblyopia.Methods The fractional anisotropy(FA) ,average diffusion coefficient(DCavg) ,neural fiber tracts of corpus callosum and optic radiations between 30 persons with ametropic am-blyopia and 30 healthy ones were compared by diffusion tensor imaging(DTI) with tractography(DTT) ;Thevisual cortical activity induced by plane-and stereo-figure in 10 of the 30 ametropic amblyopic persons and 10of the 30 healthy ones were compared by use of blood oxygenation level dependent-functional magnetic reso-nance imaging(BOLD-fMRI) .Results The activation in the regions of dorsal visual cortex decreased ordisappeared in ametropic amblyopic group compared with the normal group,DCavg values in the body ofcorpus callosum was higher,And FA values in bilateral optic radiations were lower.All these results have statis-tical significance and coincide with the decreased neural fibers of the optic radiations,body and splenium ofcorpus callosum.Conclusions The disruption in the development of corpus callosum,optic radiations and thereduced activity of the neurons or reduce synchronization among aetivity neurous in visual cortex may be majorcauses of visual cortical deficit in people with ametropic amblyopia.

目的基于大脑各皮层区的功能动力学特征受到皮层间及皮层外解剖结构连接制约的观点,探讨屈光不正性弱视视觉皮层功能损害与胼胝体、视辐射发育不良的关系。方法利用弥散张量成像(DTI)及纤维跟踪技术(DTT)比较30例屈光不正性弱视患者与30例正常人胼胝体、视辐射各向异性分数(FA)值和平均弥散系数(DCavg)值,及神经纤维投射方式;并对屈光不正性弱视组和正常组各10例行血氧水平依赖的功能性磁共振成像(BOLD-fMRI),分别在平面与立体图刺激下比较视皮层激活水平。结果与正常组比较,屈光不正性弱视组背侧视皮层区域激活范围与强度显著低于正常组,胼胝体体部DCavg值显著高于正常组,视辐射FA值显著低于正常组,连接枕叶与顶枕联合皮层的胼胝体纤维数与视辐射纤维数明显减少。结论胼胝体后部和视辐射发育不良,视皮层神经元活动减少或神经元同步化活动减少可能是弱视视皮层功能损害的主要机制。
