Objective To evaluate the efficacy of modified ultrafiltration on concentrating diluted blood and studied the change of concentrations of some cytokins.Methods Forty children affected with congenital heart disease was divided into modified ultrafiltration group (MUF group, n=20) and control group (C group, n=20) randomly. Hematocrit at about 20% and colloid pressure at about 1.33kPa were maintained during the cardiopulmonary bypass. In MUF group, modified ultrafiltration was used after cardiopulmonary bypass....

Objective To evaluate the efficacy of modified ultrafiltration on concentrating diluted blood and studied the change of concentrations of some cytokins.Methods Forty children affected with congenital heart disease was divided into modified ultrafiltration group (MUF group, n=20) and control group (C group, n=20) randomly. Results After ultrafiltration, hematocrit and the concentrations of macromolecular substances such as the concentration of IL - 8. ET. BUN and Cr were decreased significantly, but the concentration of TNF did not change obviously. Conclusion Using modified ultrafiltration after cardiopulmonary bypass can concentrate blood cell and protein swiftly, and harmful medium molecular substances were filtered out at the same time.

目的研究改良超滤法浓缩血液的效果及其对体内部分细胞因子(TNF、IL-8、ET)的影响。方法40例小儿先心病患者,随机分为两组(实验组和对照组),每组20例。体外循环中维持血球压积20%,胶体渗透压1.33kPa左右。实验组病人在停机后,拔管前行改良法超滤。结果超滤后,病人的血细胞及蛋白等大分子物质的浓度明显提高,而IL-8、ET和尿素氮、肌酐这些中小分子物质的浓度经白蛋白校正后明显降低,TNF浓度无明显变化。结论应用改良超滤法可以迅速浓缩血液中的细胞和蛋白,在滤出水份同时,还能有效的去除多种有害中分子物质。

Objective To summarize the clinical experience of blood saving technique employed in open heart surgery on children under 10kg of weight. Methods Ninety two young children with congenital heart disease were divided into two groups, study and control groups. The membrane oxygenator, which was primed less volume, and modified ultrafiltration, which can concentrate the blood after cardiopulmonary bypass (CPB), were used in study group. Results Compared to control group, much less packed red blood cells and free frozen plasma, decreased from 343±73ml, 285±138ml to 220±77ml, 162±72ml, respectively, were used in study group during CPB. And the volume of packed red blood cells, which were used after CPB, was decreased from 356±140ml to 275±128ml in study group. Conclusion During CPB, the synthetical management, including decreasing priming volume, using modified ultrafiltration, decreasing blood loss, stanching bleeding, ameliorating blood damage and so on, should be used to save blood.

目的 探讨婴幼儿先天性心脏病手术减少围术期用血量的体外循环方法。方法 将10kg 以下先天性心脏病手术患儿92 例分为实验组和对照组,实验组使用预充量小的氧合器,并采用改良超滤法。 结果 同对照组相比,实验组可以将体外循环预充所用的少浆血和血浆用量分别从343 ±73ml 和285 ±138ml 降低到220 ±77ml 和162 ±72ml,还可以把术后少浆血用量从356 ±140ml 减少到275 ±128ml。 结论 体外循环中要采用综合性措施减少围术期用血量,应尽量减少预充量,并进行血液浓缩;同时还要减少出血,加强止血;减少血液破坏,才能最大限度的减少用血量。

Results CO,CI,SV,SVI at post-MUF increased from 4.18±0.61,2.77±0.53,49.3±15.4,31.9±11.1 to 5.17±0.58,3.26±0.34,59.0±14.2,36.9±8.6;(allP<0.05....

目的 观察改良超滤对成人心瓣膜置换术后心功能的影响。方法 用Swan -Ganz导管及Baxter心排量监测仪动态观察CPB前后及MUF后CO、CI、SV、SVI、LVSWI、RVSWI、MPAP、PAWP、SVR、PVR、CVP及MAP。结果 超滤前后CO、CI、SV及SVI分别从 4.18± 0 .6 1、2 .77± 0 .5 3、49.3± 15 .4、31.9± 11.1上升到 5 .17± 0 .5 8、3 .2 6± 0 .34、5 9.0± 14.2、36 .9± 8.6 (P <0 .0 5 ) ;MAP、LVSWI从 10 .6± 0 .6、3.18± 1.2 1上升到 12 .4± 1.0、4.19± 0 .96 (P <0 .0 1) ;SVR、PVR、MPAP、RVSWI无明显变化。结论 改良超滤在成人心脏直视术中 ,同样能通过减轻心肌水肿 ,增加心肌收缩力及舒张顺应性 ,提高心指数 ,改善心功能。
