The recovery of cardiac hemodynamic indices at 30 minutes of reperfusion for crystalloid group vs PFCE group was 86±8% vs 100±8% (P<0.01 )for LVSP;92±8% vs 111 ± 16% (P<0.05) for dp/dtmax;68±10% vs 105±17% (P<0.01) for dp/dtmax 171 ±36% vs 112±18% (P<0.01) for LVEDP;60±12% vs 93±11% (P<0.01 )for AoF .The ultrastructural damage was less common and extensive in PFCE group than in crystalloid group. The results clearly showed that PFCE could protect ischemic heart better than crystalloid cardioplegia.

Accumula. tion of Na~+, Ca~(2+) was reduced by prior depleting of glycogen. These data suggested that the protective effects of depleting myocardial glycogen were associated with less production of lactate and H~+ which activated Na~+/H~+ exchange.

本工作在离体大鼠等容收缩心脏模型上观察耗竭心脏细胞内糖原对心肌缺血再灌注损伤的影响。心脏富氧灌流30 min后,随机分为三组:Ⅰ、富氧组:富氧灌流75min。Ⅱ、对照组:常规K-H液富氧灌流15min,旷置30min,再灌流30min。Ⅲ、耗竭糖原组:先用充N_2(95%N_2:5%CO_2)的K-H液灌流15min,余同组Ⅱ。结果表明。耗竭心肌细胞内糖原可提高再灌注后心脏血液动力学的恢复;冠脉流出液中LDH活性及心肌组织MDA含量降低,线粒体及胞浆液中GSH-Px有较高的活性;心肌组织Na~+,Ca~(2+)超负荷减轻。说明耗竭心肌细胞内糖原可通过减少细胞内H~+的生成抑制Na~+/H~+交换,从而明显减轻心肌缺血再灌注损伤。

The results showed that cardiac output (CO), stroke volume (SV), cardiac indx (CI) and stroke voume index(SVI) were lower than those of the normal persons, but the total peripheral resistance (TPR), heart rate (HR) and the average aortic blood pressure (ABP) were unchanged. 对45例肝炎后肝硬化患者作了心脏血流动力学方面的检测,结果发现每分输血量(Co)、每搏输血量(SV)、心脏指数(CI)、心搏指数(SVI)均低于正常对照组(P<0.05~0.001),总外周阻力(TPR)无明显变化(P<0.05);Co、SV、CI、SVI TPR在Chil A、B、C级之间有显著差异(P>0.05~0.001),其关系:Co、SV、CI、SVI为 A级>B级>C级,TPR为C级>B级>A级。还发现CO、SV、SV、CI、TPR与门脉高压程度有明显关系(P<0.05~0.01)。并认为肝炎后肝硬化患者心脏血液动力学变化可能与心肌收缩功能损害有关。
