Objective To study the effect of early nutritional intervention on the serum insulin-like growth factors(IGF1),insulin-like growth factor binding protein 3(IGFBP3), intestinal development and catch-up growth in rats born with intrauterine growth retardation(IUGR).Methods Twenty-four IUGR female rats and 8 normal female rats were divided randomly into 4 groups: 1.IUGR control,2.IUGR low-protein diet group,3.IUGR high-protein diet group,4.normal control group.At 4th week of life,the serum IGF1,IGFBP3, body weight,length, and intestinal weight, length, intestinal constitution were measured in all groups.Results At the 4th week, the serum IGF1, IGFBP3, intestinal villi height and villi absorbing area in IUGR high-protein diet group were significantly higher than those in normal control group and IUGR control group( P<0.05 or P<0.01). The intestinal weight, length and body weight,length were not significantly different compared with normal control group(P>0.05).Conclusions The high-protein diet of early nutritional intervention during 4th week of life is helpful for catch-up growth by promoting the intestinal development in rats born with IUGR. The serum IGF1 is a sensitive index to the catch-up growth.

目的 探讨早期营养干预对宫内生长迟缓 (IUGR)幼鼠血清胰岛素样生长因子 (IGF)和小肠发育及生长追赶的影响。方法 将 2 4只IUGR新生雌鼠和 8只正常新生雌鼠随机分为 4组 :IUGR模型组 ,IUGR低蛋白组 ,IUGR高蛋白组 ,正常对照组。于生后 4周时检测各组大鼠血清IGF1、胰岛素样生长因子结合蛋白 3(IGFBP3)浓度、体重、身长和小肠重量、长度及肠黏膜组织结构变化。结果  4周时IUGR高蛋白组血IGF1、IGFBP3和小肠黏膜绒毛高度、吸收表面积均显著高于对照组和IUGR模型组 (P <0 .0 5或P <0 .0 1) ;小肠重量、长度和体重、身长与对照组比较无显著性差异 (P均 >0 .0 5 )。结论 生后头 4周予高蛋白饮食早期营养干预IUGR幼鼠 ,可通过促进小肠发育达到满意的体格生长追赶。血清IGF1是反映生长追赶的灵敏指标

Objective To look for an appropriate dietary pattern of early nutritional intervention, which does not only meet the need of catch-up growth but also avoids or reduces the incidence of insulin resistance (IR) in adulthood of rats born with intrauterine growth retardation (IUGR). Methods The model of IUGR in rats was established by maternal nutrition restriction. Sixty newborn female rats with IUGR were randomly divided into 5 groups: (1) IUGR control group fed with common diet. (2) IUGR high-carbohydrate diet group. (3) IUGR high-fat diet group. (4) IUGR high-protein diet group. (5) IUGR low-protein diet group. The IUGR newborn rats were breast-fed for 3 weeks, while the mother rats were fed with the above different diets with the same caloric amount. Twelve normal newborn female rats served as a normal control group and were fed with common diet. All newborn rats were fed routine diet starting from the 4th week of experiment. The body weight, perirenal fat weight and the serum leptin, blood glucose, insulin concentration were measured and the insulin sensitive index (ISI) were calculated at the 4th week and the 12th week of life. Results The IUGR rats fed with high-protein diet showed a catch-up growth without the increase of perirenal fat at the 4th week, a normal level of perirenal fat, leptin and ISI at the 12th week compared with the normal control group, and did not show IR. The groups fed with high carbohydrate diet and high fat diet also showed a catch-up growth, but did the same as the IUGR control group in other aspects, they all showed increased perirenal fat, higher levels of leptin, lower ISI and IR at adulthood. The group fed with low protein diet did not show IR, but kept a small body size with increased perirenal fats. The serum leptin level of rats was positively correlated to body weight at the 4th week of life and was positively correlated to the weight of perirenal fat and negatively correlated to ISI at the 12th week of life. Conclusion High protein diet is an appropriate early nutritional intervention for rats with IUGR. The serum leptin level at the 12th week of life seems to be an index of IR in adult rats with IUGR.

目的 探寻既能保证宫内生长迟缓 (IUGR )大鼠生长追赶又可避免或减轻其在成年期产生胰岛素抵抗 (IR)的早期营养干预措施。方法 用孕鼠全程饥饿法建立IUGR大鼠模型。IUGR新生雌鼠60只随机分为五组 :( 1)IUGR对照组给予常规饲料 ;( 2 )高碳水化合物组 ;( 3 )高脂肪组 ;( 4 )高蛋白质组 ;( 5 )低蛋白质组。正常新生雌鼠 12只为正常对照组。幼鼠在 3周喂乳期间母鼠分别摄食上述饲料 ,第 4周起各组幼鼠均以常规饲料饲养。各组新生鼠于第 4周、12周分别测定体重、肾周脂肪重量、血清瘦素、血糖、胰岛素并计算胰岛素敏感指数 (ISI)。结果 IUGR高蛋白质组在 4周呈现不伴肾周脂肪增加的体重追赶生长 ,在 12周肾周脂肪不增多 ,瘦素、ISI均与正常对照组比较差异无显著性 ,不出现IR。结论 生后哺乳期高蛋白饮食继后恢复正常饮食是早期营养干预IUGR的合理措施。 12周时血清瘦素水平可作为观察IUGR大鼠成年后发生IR的一个生化指标。

Studies showed that early "nutritional programming" would results in long-term eff ects on the body growth and organic function, and gastrointestinal development i s closely related to the body development as well. The authors aimed to study t he effect of early nutritional interventions on serum IGF1, IGFBP3, intestinal d evelopment and catch-up growth of pups with IUGR by using diets with different protein and caloric levels during the first four weeks of life. Meth ods An IUGR rat model was established by maternal nutrition restri ction during pregnancy. Thirty-two IUGR female pups were divided randomly into 4 groups(8 pups in each group)and eight normal female pups as control. The groups and interventions were (1) Normal control group(C group); (2) IUGR contro l group(S group), (3) IUGR low-protein diet group(SL group); (4) IUGR high-pro tein diet group (SH group); (5) IUGR high-caloric group( SA group). The serum IGF1, IGFBP3, body weight, body length, and intestinal weight, length, intestina l villi height(VH), crypt depth(CD), villi absorbing area(VSA), mucous thickness (MT) were measured atthe 4 th week of life. Results (1) At the 4 th week, the serum IGF1 (724.0±153.5 ng/ml), IGFBP3 (9.6 9±3.13 ng/ml),and VH (416.9±46.3 μm), VSA (115.9±24.0×103μm2), MT (583.9±68.5 μm) in the SH group were significantly higher than those of no rmal control group (539.4±198.4 ng/ml, 4.77±2.98 ng/ml and 322.1±25.8 μm, 85.8±17.8×103μm2, 480.0±61.5 μm) and IUGR control group(P< 0.05). The intestinal weight (1.91±0.16 g) and length (80.67±9.47 cm) in the SH group was not significantly different from the normal control group (2. 24±0.22 g and 74.77±9.06 cm, P>0.05). The SH group showed the fastest catch-up growth. Their body weights (40.14±11.03 g) at the 3 rd week and body lengths (23.61±0.49 cm) atthe 4 th week of life reached the no rmal ranges of the control group (44.65±5.36 g and 23.10±1.42 cm, P>0 .05). (2) The serum IGF1 (346.7±85.3 ng/ml), IGFBP3 (1.4±0.21 ng/ml), bo dy weight (21.41±3.54 g) and body length ( 15.96±1.29 cm) and the most of intestinal indexes in the SL group were markedly lower than other groups at t he 4 th week of life (P<0.05). Conclusion The serum IGF1 was a sensitive marker to reflect the catch-up growth and nutritiona l status, and IGF1 was positively correlated with the intestinal development and body growth. When given different nutritional interventions during the first f our weeks of life, high protein diet is more helpful for the IUGR catch-up grow th by promoting the intestinal development and the absorption of nutrition.
