Objective To develope a new type of GBR barrier-composite chitosan membranes for clinical use in oral-cranio-maxillofacial surgery.Methods The composite chitosan barrier was made by chitosan gel and micro-crystalline coral, and submmited to physi-chemical assay and biocompatible test in vitro. Objective To develope a new type of GBR barrier-composite chitosan membranes for clinical use in oral-cranio-maxillofacial surgery.Methods The composite chitosan barrier was made by chitosan gel and micro-crystalline coral, and submmited to physi-chemical assay and biocompatible test in vitro. Then they were implanted into bony defects in rabbis' jaw with control sites, in which no barrier was implanted. Healing of the bony defects was assessed by means of Tetracycline fluorescent label, radioisotope tracer, autoradiography, and multi-media computer estimate.Results The newly developed composite chitosan membrane had good biocompatible and better bioactivities and could successfully stimulate the proliferation of osteoblasts and accelerate the healing of the cranio-maxillary bone defects in rabbits. The statistical analysis showed that there were significant differences on bone regeneration between experimental and control groups (P< 0.05 ).Conclusion The newly developed composite is a hopeful GBR material for injured bone regeneration, and is worth to be further developed into a products for reconstructing the oral-cranio-maxillofacial bony defects.

目的 开发研制新型骨引导再生膜 -壳聚糖复合膜用于口腔颌面外科骨缺损的修复。方法 壳聚糖复合膜由可降解壳聚糖凝胶和珊瑚微晶复合而成 ,成膜经理化性能及生物相容性测试后植入家兔的实验性颅颌骨缺损中 ,设置对照组 ;采用不脱钙组织切片、四环素荧光示踪、放射性同位素标记、放射自显影观测以及多媒体骨组织计量学分析等手段对该膜制品促进骨创愈合的效果进行了系统的评估。结果 该新型壳聚糖复合膜理化性能及生物相容性良好 ;体内植入与对照组相比可明显促进家兔颅颌骨创愈合 (P <0 .0 5 )。结论 新型壳聚糖 /珊瑚微晶复合膜具备良好的引导成骨活性 ,可望进一步开发成一种有效的口腔颌面骨组织引导再生膜制品。

Objective: To summarize the experiences in using titanium membrane for guided bone regeneration.Methods:Guided bone regeneration procedures employing nonresorbable titanium membrane in 30 patients with bone deficiencies were used for repairing and augmentation of the alueolar ridge.In postoperative periodic follow-up,The bone regeneration effect were observed by successive clinical and X-ray examination.Results:47 implants were inserted in the 30 patients and titanium membrane were used in the sites of the 39 implants.All the implants had ideally osseointegration at stage Ⅱsurgery and were prosthetic reconstructed successfully.Increased opaque image were noticed on the surface of the cover screw in 15 of the 39 implants in 2 months postoperatively,and 4 months postoperatively,regenerated bone must be removed by osteotome in stage Ⅱsurgery.Conclusion:Titanium membrane have ideal effect of guided bone regeneration using of the titanium can promote implant osseointegration.Some inherent flaw can be solved by correctly design and operation skill.

目的 :总结牙种植术后使用钛膜引导骨再生临床体会。方法 :对30例47枚牙种植术中发现骨缺损、骨量不足的患者采用钛膜进行骨引导再生修复骨缺损及骨增量。术后定期观察 ,对新骨生长情况进行连续临床和X线的观察分析。结果 :30例47枚牙种植术中 ,39枚种植体植入部位使用了钛膜。二期手术时种植体均已与骨组织形成理想的骨融合 ,顺利完成种植义齿修复。39枚种植体中有15枚种植体术后2个月的X线片可见到种植体封闭螺帽上方骨密度增高影。4月后二期手术切开牙龈时可见到新骨覆盖种植体表面 ,以骨凿等去除新骨后方可见到封闭螺帽。结论 :医用钛膜在种植术中应用有较好的引导骨再生作用 ,有利于种植术后骨融合期新骨的形成。不可吸收性膜的一些固有缺陷可通过临床正确的设计关在术中严格按照操作要点进行手术 ,可获得理想的骨再生效果
