目的研究在高血压脑出血手术中行颅内迷走神经减压术对病人血压的影响。方法对14例高血压脑出血病人在清除脑内血肿后,即刻在同侧乳突后钻孔,行迷走神经微血管减压术。结果手术后3例血压完全正常,6例在手术1周内血压接近正常,2例在手术2周内血压接近正常,3例血压改善不明显。结论颅内迷走神经血管减压可以治疗顽固性高血压,避免高血压脑出血术后的再出血,而且为原发性顽固性高血压的治疗提供了一条新的途径。

Objective The aims of the article are to comparative study the blood pressure changes by intracranial vagal neurovascula decompression and traditional treatment methods, and to understand the true mechanism and effective surgical treatment method in neurogenic hypertension.Methods 1、Clinical study:36 cases suffered hypertensive intracerebral hemorrhage (HICH) were administered and operated on vagal neurovascular decompression immediately after evacuation of HICH. The blood pressure changes of the patients were investigated and followed up. 2、Retrospective Study:281 discharged patients with HICH treated by traditional methods including surgical and medicine in the last years were followed up as a comparative study. Rusults 1、Clinical study:The levels of the blood pressure in 11 of 36 patients became normal, 12 approach normal level, 7 no changes after vagal neurovascular decompression, except 7 died of HICH after operation. 2、Retrospective study:Retrospective analysis revealed that the blood pressure in patients treated with traditional methods was similar as it before intracerebral hemorrhage. Conclusions Left vagal neurovascular compression may arouse the hypertension and the vagal neurovascular decompression may decrease the level of hypertension in patients with HICH.

目的分析颅内迷走神经血管减压手术与传统方法对高血压脑出血患者血压的影响,探讨高血压的发病机理和外科治疗的有效性。方法1、临床研究:选择36例高血压脑出血清除脑内血肿后行迷走神经微血管减压术的疗效。2、回顾性研究:对281例高血压脑出血后采用传统方法治疗的病人进行回顾性研究。观察用传统的内科和外科方法治疗后的血压变化并进行比较。结果1、临床研究:迷走神经微血管减压术的36例中除7例(19%)死亡外,在29例存活的患者中10例(34%)血压正常,12例(41%)血压接近正常,7例(24%)与手术前无明显变化。2、回顾性研究:在出血后采用传统的内、外科方法治疗的281例患者中,85%的血压无明显变化,而后期血压基本正常者(15%)均为出血前无明确高血压病史者。结论左侧颅内迷走神经血管减压确能降低顽固性高血压,其有效率>75%。采用传统方法治疗的患者中有15%血压能够自然恢复到基本正常的水平。但是在出血前有明确高血压的患者无一例血压能够自然恢复正常。
