Objective To study the compression factor and clinical manifestation of the compression of the palmar cutaneous branch of the median nerve.Methods Anatomic study was done on both sides of 2 cadavers and 6 cases of hand injury in the debridement,the origin, course, branch of the palmar cutaneous branch of the median nerve were observed. From 1995 to 1998, 12 patients of compression of the palmar cutaneous branch were treated by local blockade injection. Among them, there were 8 males and 4 females, aged from 23 to 65 years and the course of disease ranged 3 to 12 months.Results The palmar cutaneous branch of the median nerve was (1.3±0.1) mm in diameter, it could be pulled when the wrist dorsi extention. All cases showed good recovery of hand function and no recurrence after 4 to 12 months follow up.Conclusion The palmar cutaneous branch compression syndrome is closely related to the local anatomy. The diagnosis is definited according to the clinical symptoms and signs, and local blocking is effective on the most patients.

目的介绍腕部正中神经掌皮支卡压综合征的临床表现,提高对该病的认识水平和治疗效果。方法对2具新鲜尸体和6例手外伤的正中神经掌皮支来源、走行及分支进行解剖和观测,并采用药物封闭治疗。结果正中神经掌皮支在腕横韧带及手掌位置表浅固定,在穿出手掌固有筋膜前有不同程度的膨大。对1995年~1998年收治的12例,随访4~12个月,症状均消失无复发。结论正中神经掌皮支卡压的病因与局部解剖因素有关。其诊断依据主要为腕手掌疼痛,鱼际区皮肤感觉障碍,但无肌肉萎缩。封闭治疗效果好。

Methods 20 sides in 10 cases of upper extremity cadavers fixed by formalin were involved .Microscopic anatomy dissected posterior interosseal nerve, forearm lateral cutaneous nerve, superficial branch of radial nerve, wrist joint branch of wrist dorsal branch of ulnar nerve innervating wrist in the dorsal surface as well as anterior interosseal nerve, palmar cutaneous branch of median nerve, profound branch of ulnar nerve and its main trunk of wrist branch innervating wrist joint in palmar side . Results Wrist joint at dorsal part was principally innervated by posterior interosseal nerve .It was also innervated by forearm lateral cutaneous nerve, superficial branch of radial nerve, wrist dorsal branch of ulnar nerve. Wrist joint at palmar side was innervated by anterior interosseal nerve, palmar cutaneous branch of median nerve and profound branch of ulnar nerve.Conclusion Treatment of intractable pain of wrist joint by denervation is mainly indicated in wrist pain at dorsal side.

目的 观察支配腕关节神经的来源、直径、数目及其行径 ;为去神经支配治疗腕关节疼痛提供解剖学资料。方法 对 10具 2 0侧福马林固定的上肢标本 ,在手术显微镜下解剖并观察骨间后神经、前臂外侧皮神经、桡神经浅支、尺神经腕背支支配腕关节背侧的腕关节支 ;骨间前神经、正中神经掌皮支、尺神经深支及其主干支配腕关节掌侧的关节支。结果 骨间后神经是支配腕关节背侧神经的主要来源 ;前臂外侧皮神经、桡神经浅支、尺神经腕背支也发支支配腕关节背侧。骨间前神经、正中神经掌皮支、尺神经深支发支参与支配腕关节的掌侧。结论 用去神经支配的方法治疗腕关节顽固性疼痛主要适用于腕背侧的疼痛。
