In patients undergoing total knee replacement we carried out a longitudinal cohort study to determine the efficacy and safety of a postoperative autologous blood reinfusion system, as an alternative to homologous, banked blood transfusions.
      
Fifty patients received reinfusion of unwashed, filtered, shed blood, supplemented with banked blood transfusions as required.
      
The purpose of our study was to determine the effectiveness of a postoperative autologous blood reinfusion system as an alternative to homologous, banked blood transfusions in total knee arthroplasty.
      
In 30 of these patients (group A), a reinfusion system of unwashed salvaged blood was applied, and they were supplemented postoperatively with banked blood transfusions when required.
      
A control group of 30 patients (group B), in whom standard suction drains were used, received one unit of homologous banked blood transfusion intraoperatively and additional blood transfusions postoperatively when required.

Objective To explore effects of ultraviolet irradiation on storage of red blood cell and the safety and feasibility of ultraviolet irradiation on banked blood. Methods Paired blood samples were used for contrast. Changes in erythrocyte fragility, blood ion, erythrocyte ATP enzyme activity and erythrocyte immune functions were tested 72 h and 120 h before and after ultraviolet irradiation, respectively. Results The erythrocyte fragility did not change significantly after storage and ultraviolet irradiation; ATP enzyme activities of red blood cells were increased as compared with those of control group (P< 0.05). Rosette rate of red blood cell C 3b receptor (RCR) and rosette forming effective rate (RFER) of red blood cell C 3b receptor decreased sharply with the prolonged storage time (P< 0.001), but were significantly increased as compared with the controls (P< 0.01). Conclusion Ultraviolet irradiation can improve the functin of red blood cells and enhance effectiveness of blood transfusion.

目的 了解紫外线照射储存血后红细胞在不同时期血液的理化指标及免疫学指标的变化 ,以探讨紫外线照射储存血的安全性及可行性。方法 采用成对血标本对照进行紫外线照射前后及储存 72、12 0h后测定红细胞渗透脆性 ,血浆K+ 、Na+ 、Cl-离子含量和红细胞ATP酶活力及红细胞免疫功能的变化。结果 储存血在保存前后与照射前后红细胞脆性均无显著变化。照射后储存血的红细胞ATP酶活力与对照组相比有所升高 (P <0 .0 5 ) ;红细胞C3 b受体 (RCR)花环率与红细胞C3 b受体花环促进率 (RFER)均有显著升高 (P <0 .0 1)。结论 紫外线照射储存血可以改善红细胞膜的代谢 ,增强红细胞的免疫功能

AIM: To explore the effects of ultraviolet irradiation on the function of lymphocytes in banked blood. METHODS: Changes of lymphocyte subgroups, lymphocyte activity, lymphocyte transformation, interleukin 2 and its receptor were tested before and after ultraviolet irradiation, and after storage for 72 h separately. RESULTS: Ultraviolet irradiation inhibited the lymphocyte activity. As compared with that of the control group, the content of interleukin 2 decreased significantly with prolonged storage time( P <0.05). Ultraviolet irradiation markedly inhibited the lymphocytes transformation( P <0.01). CD3 +, CD4 + and CD8 + lymphocyte subgroups in banked blood decreased as compared with those of prestorage in the two groups,but there was no significant difference between the two groups. CONCLUSION: Ultraviolet irradiation can inhibit lymphocytes activity and lymphocytes transformation and reduce the accumulation of cytokine in banked blood.

目的 :研究紫外线照射对储存血中淋巴细胞功能的影响 .方法 :在血标本经紫外线照射前后及储存 72h后测定淋巴细胞亚群及活性、细胞因子 .结果 :紫外线照射储存血可抑制淋巴细胞的活性 .照射后随储存时间的延长 ,血液中IL 2水平与对照组相比明显降低 (P <0 .0 5 ) ;经紫外线照射后淋巴细胞转化率较对照组明显降低 (P <0 .0 1 ) ;对照组与照射组血液在放置 72h后CD3+ ,CD4 + ,CD8+ 细胞亚群均有明显下降 ,但无显著差异 .结论 :紫外线照射储存血可抑制淋巴细胞的活性 ,对淋巴细胞的增殖能力有明显抑制作用 ;可减少细胞因子的产生和积累

目的对外科围手术期患者、稀有血型患者/大出血患者实施自体储血式输血。方法患者术前3d以上采用一次性或“蛙跳式”采血法采集血液,置4℃贮血冰箱内保存。术中或术后使用。结果138例患者手术中或手术输注自体储存血,手术后Hb、Hct轻微下降。结论自体储血式输血只要掌握好适应证,对手术患者具有安全、经济实用、疗效好的价值,而且可以节约血源,值得推广。
