The results showed the satisfactory recording could be taken as early as 19 weeks and various -cardiac structure were satisfactorily recorded in 1-59 fetuses with a success rate of 94%....

To assess the applicability and reliability o f real-time two-dimensional echocardiography in visualizing the cardiac anatomies of Chinese fetuses and to provide a methodology for further diagnosis of fetal heart diseases, we examined 170 fetuses from 16 to 42 gestational weeks using realtime two-dimensional echocardiography . The results showed the satisfactory recording could be taken as early as 19 weeks and various -cardiac structure were satisfactorily recorded in 1-59 fetuses with a success rate of 94%. The four-chamber, long-axis and short-axis view at the- level of great arteries were chosen to display fetal cardiac anatomies with the success rate of 90% 71% and 40% respectively. The satisfactory recordings were not obtained in 11 fetuses and among the factors contributing to such a failure were maternal obesity, excessive movements of fetuses and so on.

为评价二维超声心动图显示胎儿心脏结构的可行性和可靠性,为进一步应用超声心动图诊断胎儿心血管系统异常提供方法学基础,应用二维超声心动图检查了170例16~42孕周的胎儿。结果表明:二维超声心动图显示胎儿心脏满意图象可始于19孕周。在159例胎儿中获得了满意的心脏图象,成功率为94%。选用四腔心切面、心脏长轴切面和大动脉根部水平短轴切面显示胎儿心脏结构,成功率分别为90%,71%和40%。在11例胎儿中未能记录到满意心脏图象,原因为胎儿过小,胎动过频和母体肥胖等。这一研究表明:二维超声心动图,是评价胎儿心脏结构的可行和可靠技术。

In order to diagnose cardiovascular malformation in fetus and evaluate the importance of echographic diognosis,M-mode, Two-dimensionalmode and Pulsed Doppler Echocardiography were used in combined manner for examining the 310 fetuses after 16 weeks of gestation. 11 cases of cardiovascular malformation have been detected. All cases with the malformation were confirmed at autopsy or interview after delivery. 为了在产前能够诊断胎儿心血管发育异常,评估其超声诊断价值,我们联合应用M型及二维超声心动图对孕16周以上的孕妇310例进行产前检查,其检出先天性心脏病胎儿11例,经引产尸解及生后随访证实。并发现从有高危因素的孕妇中检出率(5.5%)明显高于无高危因素的孕妇(0.7%),两组检出率有显著差异(P<0.05)。本文认为对具有高危因素的孕妇产前应用超声心动图检查胎儿心脏,诊断胎儿心血管发育异常有重要的实用价值。

Seven cases of cardiac anomaly were diagnosed in 364 cases of fetal heart examination by echocardiography from Feb 1992 to Nov 1994. Six cases were anatomical maldevelopment and one was cardiomyopathy and all were confirmed by post partal examination. Seven cases of cardiac anomaly were diagnosed in 364 cases of fetal heart examination by echocardiography from Feb 1992 to Nov 1994. Echocardiography showed that maldevelopment of cardiac anatomy and cardiomyopathy indace hemodynamic chang, dilatation of cardiac chamber, wall thickening and depression of cardiac function. There were significant differences of atrioventrcular orifice and great vessel blood flow velocity from normal fetal heart of the same gestation-al age. Echocardiography is an impostant tool in early diagnosis of fetal heart disease and pathophysi-ological study.

1992年2月至1994年11月应用彩色多普勒超声心动图检测胎儿心脏364例。检出胎儿心脏病7例,其中心脏大血管发育异常6例,心肌病1例。结果显示心脏大血管发育异常及心肌病可使胎儿血流动力学发生明显改变。彩色多普勒超声评价各瓣口及大血管血流速度,速度积分、压力阶差等参数与正常同龄胎儿比较有明显差异。结论彩色多普勒超声心动图对心脏大血管异常血流的显示提供了胎儿血流动力学在胎儿心血管病时的诊断依据,表明胎儿心血管病对胎儿心脏结构及功能的影响。
