The results showed that pre-PSG values of P_(0.1), RR and P_(0.1)/VT/Ti in the OSAS patients were signlficantly higher than those in the snoring patients,while VT,Ti, Te and Ttot values were lower.In the first night,the post-PSG P_(0.1) value in the OSAS patients increased markedly as compared with the pre-PSG, After overn ight nasal CPAP therapy,the respiratory disorder index in the OSAS patients decreased markedly,the nadir SaO_2 increased markedly,but the post-PSG P_(0.1) value did not increase significantly. It is concluded that,before sleep,OSAS patients exhibit a higher respiratory drive and a shallow and frequent breathing pattern as compared with simple snoring patients. After nocturnal sleep,the reSpiratory drive of OSAS patients increases significantly,the breathing pattern be-comes more shal low and frequent.Nasal CPAP may effectively relieve the sleep apnea and hypopnea as well as the restulting hypoxemia and therefore correct the changes in breathing pattern and respiratory drive through noctumal sleep in patients with OSAS.

为了解经鼻持续性气道正压(nCPAP)通气治疗对阻塞性睡眠呼吸暂停综合征(OSAS)患者呼吸中枢驱动性的影响,研究了20例无二氧化碳(CO_2)储留的OSAS患者(O组)及20例单纯鼾症患者(S组)夜间睡眠前后呼吸方式及口腔阻断压(P_(0.1))的改变,并观察了nCPAP治疗对OSAS,患者呼吸方式及P_(0.1)的影响。结果显示:O组患者睡前的P_(0.1)、呼吸频率、有效吸气阻抗明显高于S组,潮气量则显著低于S组。nCPAP治疗组患者经一夜睡眠后的P_(0.1)、每分通气量、潮气量、呼吸频率等较睡前显著增高。经nCPAP治疗后O组的呼吸紊乱指数较治疗前明显降低,夜间最低氧饱和度明显提高,P_(0.1)较睡前则无明显升高。提示OSAS患者睡前的呼吸中枢驱动性高于单纯鼾症患者,其呼吸形式为浅快呼吸;经过一夜睡眠后,其呼吸中枢驱动水平较睡前明显增高,呼吸形式更为浅快;nCPAP治疗可以有效地解除睡眠呼吸暂停及其继发的低氧血症,从而逆转睡眠前后呼吸方式和呼吸中枢驱动性的改变。

目的 客观评价临床应用适应性支持通气(ASV)的可能性。方法 采用自身前后对照的研究方法,选择行有创通气的病人11例,分别给予常规机械通气方式和ASV。常规通气方式的参数设置由医师根据病情决定,ASV的参数设置包括三个参数,即病人体重(WT)、分钟通气量百分比(MV%)和高压报警限。MV%常规设为100%(相当于每公斤体重100ml/min)。FiO_2和呼气末正压(PEEP)与常规方式相同。两种方式各持续运用1h后采集数据。对比观察两种通气方式下的潮气量、呼吸频率、气道压、心率和血压。结果 与常规通气方式相比,ASV方式下的潮气量和呼吸频率无显著性变化,对气道压、心率和血压无明显影响(P>0.05)。结论 ASV根据病人呼吸力学状况调节通气参数,能获得与常规通气近似的呼吸形式,是一种操作简便、有效而安全的通气模式。
