目的:采用高甘油三脂钳闭技术比较结构甘油三酯(STG)与长链甘油三酯(LCT)在健康志愿者中的代谢效应,以评估结构脂肪乳剂代谢特征。方法:选择16位健康志愿者进行前瞻性随机交叉对照研究。采用改良的高甘油三酯钳闭技术,维持血浆甘油三酯浓度在4mmol/L水平达180分钟,比较STG和LCT的代谢特征。结果:STG的血浆清除率较LCT快一倍左右,STG组血清总游离脂肪酸、游离甘油及β-羟基丁酸盐浓度明显高于LCT组,血浆脂蛋白脂酶(LPL)活性明显大于LCT组并与所输注的甘油三酯量密切相关。结论:高甘油三酯钳闭模式可重复、有效的比较不同的脂肪乳剂的代谢特征,健康志愿者血浆STG的清除率明显大于LCT,STG的水解速度明显快于LCT,血浆LPL活性增高有利于STG的快速清除

Objectives:To examine whether the thermogenic and metabolic response of structured triglyceride (STG) is different from that of long chain triglyceride(LCT) in patients with liver cirrhosis. Methods:Sixteen overnight fasted patients with liver cirrhosis were examined before and after administration of STG or LCT.The patients were randomly allocated to two treatment order groups in an open cross over study.The energy expenditure was determined with indirect calorimetry befeore and then throughout the study period.Individual free fatty acids(FFA) were analyzed by HPLC,dicarboxylic acids(DC) in urine and plasma and 3 hydroxy fatty acids (3OHFA) in plasma were analyzed by gas chromatography mass spectrometry. Results:The infusion of STG was found to be accompanied by two fold greater thermic effect and the respiratory quotient tended to be lower in comparison with the infusion of LCT.Infusion of STG was associated with appearance of DC and 3 OHFA in plasma but a slightly above the detection level following administrating of LCT. Conclusions:In patients with liver cirrhosis administration of STG in comparison with LCT is accompanied by a more marked hydrolysis as well as a two fold greater thermogenic response.Fat emulsions containing medium chain triglyceride ought to be administrated at a relatively slow rate in order to avoid potential adverse effects due to an over loaded metabolism.
