Objective To evaluate the feasibility and efficacy of real-time three-dimensional echocardiography in ventricular septal defects (VSD).Methods Five patients with VSD were examined by real-time three-dimensional echocardiography and two-dimensional echocardiography.Three were membranous septum defects,one was membrane-surranding septum defect,one was below branch septum defect.Results The stereoscopic views of VSD were displayed clearly by real-time three-dimensional echocardiography,including shape,position,area and spatial relations of VSD.Conclusions Compared with two-dimensional echocardiography,real-time three-dimensional echocardiography could reflect the true stereoscopic structure of VSD.It is also better than former three-dimensional voxel imaging of echocardiography in requiring quick images of the ventricular defects real-time.

目的 探讨实时三维超声心动图在室间隔缺损诊断中的临床应用价值。方法 对 5例室间隔缺损患者分别进行二维和实时三维超声检查。其中 3例为膜部室间隔缺损 ,1例为膜周部室间隔缺损 ,1例为干下室间隔缺损。结果 实时三维超声心动图能清晰显示室间隔缺损的形态、部位以及毗邻关系。结论 与二维超声心动图比较 ,实时三维超声心动图反映了室间隔缺损的立体空间结构。与以往三维重建超声相比 ,实时三维超声可迅速得到室间隔缺损的三维图像。

Objective to investigate the value of real-time three-dimensional echocardiography in the diagnosis of cardiovascular diseases.Methods Philips Sonos 7500 was used to detect ninety-eight patients(52 male,46 female,mean age 20.92±19.16 years)in which 20 normal and 45 congenital heart disease and 26 valvular disease,7 other heart diseases.Results Real-time three-dimensional echocardigraphy can visualize technically satisfactory dynamic morphology views of heart;can detect the position,size and shape of atria or ventricular septum defect,special relation to the surrounding tissue;can detect variform malformation of complex congenital heart disease in surgical views,identify aorta and pulmonary artery; visualize,valvular apparatus and their defects.Conclusion Real-time three-dimensional echocardiography can provide adequate valuable information for accurate diagnosis of cardiovascular disease.

Objective To explore the value of real-time three-dimensional (RT-3D) ultrasonography in quantitative and qualitative diagnosis of carotid atherosclerosis. Methods Twenty-two patients with carotid atherosclerosis were examined by the RT-3D ultrasonography and the two-dimensional (2D) ultrasonography. The area and the volume of plaques were calculated. Results 2D and RT-3D ultrasonography could display the location, structure and shape of the carotid atherosclerotic plaque. RT-3D ultrasonography could reveal the sterotic structure of the plaque, and display the posterior structure of the calcific plaque which could not be revealed clearly by 2D ultrasonography. The maximal areas of plaques derived from RT-3D ultrasonography and 2D were (0.34±0.16) cm2 and (0.19±0.12) cm2 respectively (P<0.01). There was a high correlation between the maximal areas of plaques by 2D and by RT-3D ultrasonography (r2 =0.65,P<0.01). In addition, RT-3D ultrasonography could measure the volume of plaque (0.18±0.11) cm3. Conclusion RT-3D ultrasonography provides a new method in quantitative and qualitative diagnosis of carotid atherosclerosis.

目的探讨实时三维超声技术在颈动脉粥样硬化斑块定性和定量中的应用价值。方法对22例颈动脉粥样硬化患者进行二维和实时三维超声检查,观察颈动脉及斑块的形态,测量斑块面积以及三维体积,并比较实时三维超声和二维超声测量结果。结果二维及实时三维超声均可以显示斑块的位置、形态及结构,三维超声可显示斑块的立体形态,在钙化斑块也可显示远场的结构。实时三维超声测量斑块最大面积为(0.34±0.16)cm2,二维超声测量斑块最大面积为(0.19±0.12)cm2,测值差异具有统计学意义(P<0.01),两种技术测量的斑块面积存在高度正相关(r2=0.65,P<0.01)。三维超声定量斑块的体积为(0.18±0.11)cm3。结论实时三维超声在颈动脉粥样硬化斑块的定性及定量方面提供了新的方法。
