本例按照细胞形态(含组化)、免疫表型和细胞遗传学改变诊断为急性早幼粒细胞白血病。应用国产维甲酸诱导分化疗法取得完全缓解。应用光镜和电子显微镜观察病态细胞的演化规律,提出投药后l~2周以临床和血像有无改善作为本疗法取舍的时机。认为即使诱导分化取得缓解,但仍不能证明细胞逆转为正常细胞,还应在诱导分化基础上进行细胞毒疗法,以减少体内肿瘤细胞负荷量,争取较长的缓解期。

Objective:To observe the change of ultrastructure of advanced ovarian serous cystadenocarcinoma cell combined of differentiation-inducing therapy by N-(4-carboxyphenyl) retinamide with conventional surgery and chemotherapy.Methods:Thirty-seven cases of advanced ovarian serous cystadeno-carcinomas in gynecology department of Yuhuang Ding Hospital of Yantai from the year 2000 to 2005 were randomly divided into two groups.To observe the change of ultrastructure of advanced ovarian serous cystadenocarcinoma cell combined of differentiation-inducing therapy by N-(4-carboxyphenyl)retinamide with conventional surgery and chemotherapy.Results:The tumor cells in group of differentiation-inducing therapy were altered to differentiation of maturity:karyoplasmic ratio was diminished,cytoplasm was abundant.intercelluar desmosome conjunction was development well and tight,mitochondrion was increase in number and morphous was regular,electron density of ground substance was increase,glycogen granule of cytoplasm was abundant,quantity of rough endoplasmic reticulum and regular morphous of Golgi complex were observed in some cells.

目的:探讨在手术和化疗的基础上联合维胺酸诱导分化治疗后,卵巢浆液性囊腺癌组织肿瘤细胞超微结构的变化。方法:选取37例晚期卵巢浆液性囊腺癌患者为研究对象,随机分为两组,了解在手术和化疗的基础上联合维胺酸诱导分化治疗后肿瘤细胞超微结构的变化。结果:诱导分化组肿瘤细胞出现明显的成熟性分化趋势:核浆比例减小,细胞浆较丰富;细胞间出现发育较好的桥粒连接、紧密连接;线粒体数量增加形态较规则,基质电子密度增加;细胞浆内糖原颗粒丰富;部分细胞中出现数量较多的粗面内质网和形态较规则的高尔基复合体。结论:诱导分化剂维胺酸可诱导和促进肿瘤细胞分化,诱导分化疗法能够改善卵巢浆液性囊腺癌肿瘤细胞的恶性生物学行为。
