对68例狼疮性肾炎(LN)患者行肾活检检查,以定性、定量的方法观察了其小管间质损害程度、间质容量变化及肾小球病变各自对狼疮性肾炎肾功能状态的影响。结果发现:(1)伴有明显肾间质损害及肾功能异常者,肾间质容量(Ⅳ)明显增加(34.7±14.3%vs14.8±4.8%△Ⅳ为21.9±10.9%);(2)肾间质容量变化与肾功能改变之间呈明显相关关系;(3)不同的肾小球病变类型、肾小球病变活动性指数及有无IgG在小管基膜上沉积对Ⅳ及肾功能均无明显影响。提示肾间质容量改变可以直接反映肾小管间质损害程度乃至肾功能的变化。在LN伴明显肾间质损害时,肾功能的变化主要与肾小管间质改变程度有关,而与同期存在的肾小球病变类型、严重程度及活动性关系不大。Ⅳ的定量分析可作为临床判断肾脏损害程度及功能状态的一项颇有价值的指标。

The re sults showed: TlN was characterized by deposition of IgG in a linear pattern along TBM and mononucle ar cells infiltrates, which suggests the production of anti-TBM antibodies. Among the mononuclearcells, most are T cells. Granuloma formation was observed in parts of TIN. B cells gradually decreasedwhile monocytes gradually increased. The renal tissue lesions show tubular atrophy and peritubular fi brosis. This model strong1y suggests that cellular immune responses play an important role in the devel opment of TIN. Ia antigen expression is higher in cells of the tubular epithelium and B0wman'S capsuleof lesion site than in control group (P<0.01). This is strong evidence that the abnormal expression ofIa antigen may be affected by the production and development of TIN.

为探讨肾小管间质性肾炎的免疫发病机制,观察Ia抗原表达与肾小管间质性肾炎损害的关系,用兔肾小管基底膜(TBM)完全福氏佐剂及百白破疫苗,免疫雌性Wistar大鼠,成功地建立了肾小管间质性肾炎(TIN)模型。我们采用免疫组化和图像分析技术,研究TIN病变中单个核细胞的特征以及Ia抗原表达和TIN之间的关系。结果显示:在TIN发生时,TBM上有线型IgG沉积,这表明有抗TBM抗体产生。肾小管间质中渗出的单个核细胞,大多数为T细胞,部分有肉芽肿形成。B细胞逐渐减少,而单核细胞由少变多。肾间质损害表现为小管萎缩和间质纤维化。这些表明细胞免疫在TIN发病过程中起着重要作用。模型组中肾小管上皮细胞和Bowman囊Ia抗原表达强度比对照组高(P<0.01),提示Ia抗原的表达可能影响TIN的发生和发展。

为客观了解益母草是否会导致肾损害 ,采用按成人等效剂量不同倍数的益母草水煎剂 ,分别持续 30天、4 5天、6 0天给大鼠灌胃 ,观察其对大鼠肾功能及肾脏组织形态的影响。结果 :益母草对肾小球无损伤作用 ,但可引起肾间质轻度炎症及少量纤维组织增生、肾小管轻度脂肪变 ,且随着剂量的增大 ,病变也相对加重。提示长期服用单味大剂量益母草 ,有可能引起肾小管、肾间质的损害
