Doppler color flow imaging (DCFI) is a promising technology for the assessment of cardiac malformation. We compared DCFI to surgical findings in418patients with cardiac malformation. There were 235 males and 183 females, aged from 3 day to 13 years (mean age 5 yrs). Among them 35% underwent cardiac catheteri-zation and/or angiocardiography. Doppler color flow imaging (DCFI) is a promising technology for the assessment of cardiac malformation. We compared DCFI to surgical findings in418patients with cardiac malformation. There were 235 males and 183 females, aged from 3 day to 13 years (mean age 5 yrs). Among them 35% underwent cardiac catheteri-zation and/or angiocardiography. This series of patients included 340 isolated anomalies and 78 complex malformations. Isolate malformations consisted of VSD (202) PDA (85), ASD(46), PS (6), RCA and RPA fistula (1). These diagnoses were 100% confirmed by surgery. The smallest detectable diameters of ASD, VSD and PDA were 0.5cm, 0.1cm and 0.4cm. Complex anomalies included TOF (44), VSD + PDA+PH(15), VSD+ASD(5), ASD+PAPVD(4), TAPVD(2), PA(2), Ebstein's anomaly(2), C-TGA(1), ASD+PDA(1), AMB+PS(1), TA(1). The DCFI diagnoses of 70 complex cardiac malformations were completely in concordance with surgery. 273 cases were operated upon directly without angiocardiography. 3 cases of critical neonatal cardiac malformation included PA + PDA (2), VSD + large PDA(1) were diagnosed by DCFI and conformed by surgery. Finally, we analysed the reasons for the misdiagnosis of 8 patients.It is suggested that surgery can be carried out without cardiac catheterization in most cases of cardiac malformation. The accuracy of DCFI is considered satisfactory in this study.

本文采用彩色多普勒血流显像(DCFI)诊断小儿先天性心脏病428例。全部患儿均接受手术治疗。将DCFI检查结果与手术发现进行了比较。本组有35%施行心导管和(或)心血管造影。418例中有340例为单一心血管畸形,DCFI与手术结果完全一致,符台率100%。复合心血管畸形78例中有70例两者结果基本相符,1例误诊(0.2%),7例有部分漏诊(1.7%)。本组有3例危重新生儿先心病,经DCFI诊断后立即施行急诊手术治疗,术中所见与DCFI诊断完全一致。观察结果显示,单一先心病及某些复合畸形,DCFI基本可取代创伤性检查而直接接受手术治疗。DCFI不仅诊断准确,而且对手术方案的选择有指导和参考价值,更重要的是为危重型婴幼儿先心病的早期诊断、及时手术治疗开辟了令人鼓舞的前景。

Color doppler flow imaging(CDFI)was used in examina- tion of 42 patients who have suspicion of deep venous thrombosis(DVT)of lower-extremities.Thirty-four(80.9%)of 42 patients were identified posi- tive by CDFI for 1 patient(2.3%)was suspectable,7 patients(16.6%) were negative.The diagnostic eritera of CDFI for deep venous thrombosis were also discussed.

本文用彩色多普勒血流图(CDFI)对42例临床疑有下肢深静脉栓塞(DVT)的患者进行了检查,其中34例(80.9%)检出 DVT,7例正常(16.6%),1例可疑(2.3%)。并对 CDFI 诊断下肢 DVT 的标准进行了探讨.

Ultrasound diagnoses of 83 breast tumors were analyzed.The echographic appearance of the tumors were classified into 3 categories:benign,malignant and intermediate type.Echography is easy in diagnosing the benign type masses and malignant type masses.But it's difficult to determine the benignity or malignancy of intermediate masses.Color doppler blood flow signals of 24 benign breast tumors are mainly manifested as type Ⅰ(8 cases) and type Ⅲ(6 cases),that is,only one vessel feeding the tumor.5 cases of benign...

Ultrasound diagnoses of 83 breast tumors were analyzed.The echographic appearance of the tumors were classified into 3 categories:benign,malignant and intermediate type.Echography is easy in diagnosing the benign type masses and malignant type masses.But it's difficult to determine the benignity or malignancy of intermediate masses.Color doppler blood flow signals of 24 benign breast tumors are mainly manifested as type Ⅰ(8 cases) and type Ⅲ(6 cases),that is,only one vessel feeding the tumor.5 cases of benign breast tumors didn't show flow signal.81%(48/59) of malignant tumors showed type Ⅳ of flow signals,that is,more than two feeding vessels concentrating towards the mass like corkscrews,The combination of echographic appearance and color doppler manifestations gave a more accurate result with an accuracy of 97.6%.The limitations and appropriate application of mammography were also discussed.

本文分析了83例乳腺肿瘤的超声诊断。肿物的灰阶超声表现有三种,良性型、恶性型和中间型。灰阶超声对良性型和恶性型肿物较易定性诊断,对中间型不易定性。24例良性肿瘤彩色多普勒血流信号主要表现为Ⅰ型(8例)和Ⅲ型(6例),即呈单条血管供血,5例未见血流信号;81%(48/59)的恶性肿瘤彩色多普勒血流信号为Ⅳ型,即有2条以上的供应血管向肿物集聚。结合肿物的灰阶超声特点和彩色多普勒表现有助于判断其良恶性,诊断符合率达97.6%。对乳腺X线摄影的局限性和合理应用进行了讨论。
