Objective:We report seven cases of phenotypic conversion of adult acute myelocytic leukemia after treatment,revealing the feature of cytogenetics and clinic.Methods:The clinical information of these seven patients are collected and the abnormality of associated chromosomes and chimeric genes are detected.Results:Shifts in FAB classification are observed in all of the seven cases,two cases also present chromosomal changes.Phenotypic conversion occurs 1~14 months (mean time 6.33 months)after the first complete remission.After the phenotypic conversion these patients become insensitive to chemotherapy.Conclusion:The patients with phenotypic conversion are insensitive to chemotherapy or induce differentiation and apoptosis therapy.We suggest that the dynamic detection of minimal residual disease or complexed chromosome abnormality or novel chromosome of these patients during the period of CR 1 should be carried out.If any positive result presented,high dose chemotherapy of hemopoietic stem cell transplantation may be helpful to prolong the survival time.It is tempting to make further investigations in this field.

目的 :通过报道 7例成人急性髓系白血病治疗后转型情况 ,揭示其细胞遗传学及临床特征。方法 :对 7例患者进行临床资料收集、分析以及染色体和融合基因检测。结果 :7例患者均有 FAB分类的形态学改变 ,其中 2例既有形态学改变 ,又有染色体变化 ;转型发生时间距第一次完全缓解时间 1个月~ 14个月 (平均 6 .33月 ) ;转型后 7例患者对化疗均不敏感。结论 :转型患者对化疗或诱导分化、凋亡治疗均不敏感 ,如能在 CR1 期动态监测微小残留病灶或发现复合染色体异常或出现新的染色体异常 ,积极采用大剂量化疗、造血干细胞移植 ,可能会延长患者的生存期。

通过综述近年有关诸多中药有效单体对肿瘤细胞诱导分化、凋亡的影响文献 ,认为中药对肿瘤细胞诱导分化、凋亡 ,为肿瘤的临床与实验研究提供了新的靶点及作用机制 ,但中药品种多、资源丰富、且中医药历史悠久 ,几千年来积累了大量宝贵经验 ,细胞诱导分化、凋亡对更加深入揭示中医药的作用机制、筛选新的抗癌药物具有十分重要的意义。

The expression of mPer2 in transfected Lewis tumor cell was detected by immunohistochemistry and flowcytometry. Result Expression of PERIOD2 protein in pcDNA 3.1-mPer2 transfected Lewis tumor cell was proved by immunohistochemistry and flowcytometry. Flowcytometry result of mPer2 expression Lewis tumor cell showed less proliferation and high rate of apoptosis. Conclusion mPer2 expression can suppress the proliferation of tumor cell and increase the apoptosis of it.

目的研究节律基因mPeriod2(mPer2)对肿瘤细胞的生长抑制及诱导分化凋亡的作用。方法体外培养Lewis肺癌细胞,使用脂质体包裹法将真核表达质粒pcDNA3.1为基础的mPer2表达质粒(pcDNA3.1mPer2)转导入Lewis肺癌细胞内;以免疫组化及流式细胞技术检测mPer2表达;通过流式细胞技术检测稳定转染的mPer2阳性表达的Lewis肺癌细胞的生长及凋亡。结果mPer2表达质粒(pcDNA3.1mPer2)转染的Lewis肺癌细胞表达PERIOD2蛋白;与对照组比较,其增殖指数下降及凋亡率增加。结论节律基因mPer2可以抑制肿瘤细胞的生长,增加肿瘤细胞的凋亡率。
