Results showed the a albumin, prealbumin and transferrin of seurm in infants group, is obviously lower than in the average contrast group, and albumin level difference is notable (P<0.05), prealbumin and transferrin level difference is very obvious (P<0.01). All the three proteins reduce their level with the decreasing of gartational age and weights. Thesebore, the albumin, prealbumin and transferrin contents of the serum can be used as the indicators of nutrition conditions of preterm infants. (Original article on page 247)

本文通过对早产儿血清白蛋白 (ALB)、前白蛋白 (PA)和转铁蛋白 (TF)的测定 ,来寻求评价早产儿营养状况的方法 ,用以指导临床。采集新生儿静脉血 2 ml,应用美国 Backman公司 CX- 5型全自动化仪 ,白蛋白用溴甲酚绿比色法 ,前白蛋白用散射比浊法 ,转铁蛋白用免疫散射比浊法。结果显示 ,早产儿血清白蛋白、前白蛋白、转铁蛋白均明显低于正常对照组 ,其中两组白蛋白水平有显著性差异 (P<0 .0 5 ) ,前白蛋白、转铁蛋白水平有非常显著性差异 (P<0 .0 1 ) ,且三种蛋白随着胎龄及体重的减小而降低。因此 ,血清白蛋白、前白蛋白、转铁蛋白水平可以作为对早产儿营养状况评价的指标。

Methods Premature infants were randomly assigned into two groups: experimental group(group A,LCP enrichment formula,n=30)and control group(group B,other formula for premature infant,n=30).Clinical symptoms had been observed continually for 4 weeks. The laboratory data were monitored at the Beginning and the end of the study in both groups. Results The concentration of fatty acid compositions of plasma :20:5 n 3, 22:5 n 3 and total(n 3)LCP were significant higher in group A(0.37±0.11) mol% ,(0.27±0.17) mol%,(2.20±0.73) mol% than that in group B(0.19±0.11) mol%,(0.15±0.07) mol%,(1.68±0.51) mol%), P <0.01. The ratio of n 6/n 3 in group A was significant lower than that in group B(4.19±0.97 vs 5.47±1.24), P <0.01.The weight, height, head circumference, thorax circumference gain in group A were as much as that in group B ( P >0.05). No significant difference of laboratory data were found between the two groups ( P >0.05). Conclusion LCP enrichment formula is suitable to premature infants. Its lipid composition is more close to that of breast milk and benefit to the brain and retina development of premature infants.

目的 对强化长链不饱和脂肪酸配方奶应用于早产儿的营养价值进行临床研究。 方法 将早产儿随机分为试验组 (强化长链不饱和脂肪酸配方奶组 )和对照组 (普通早产儿配方奶组 )各30例。比较两组生长发育和有关实验室指标 ,观察时间为 4周。 结果 试验组血清脂肪酸 2 0 :5 n- 3和2 2 :5 n- 3含量及 n- 3系不饱和脂肪酸总含量高于对照组 [(0 .37± 0 .11) m ol%与 (0 .19± 0 .11) mol% ;(0 .2 7± 0 .17) mol%与 (0 .15± 0 .0 7) m ol% ;(2 .2 0± 0 .73 ) mol%与 (1.6 8± 0 .5 1) m ol% ](P<0 .0 1)。n- 6 / n- 3比值低于对照组 ,差异有非常显著性 (4 .19± 0 .97与 5 .47± 1.2 4) (P<0 .0 1)。试验组与对照组相比 ,体重、身长、头围、胸围的增长及其他各项实验室生化指标的差异均不显著 (P>0 .0 5 )。 结论试验组配方奶的脂肪酸组成接近于人乳 ,适用于早产儿 ,有利于早产儿脑和视网膜的发育
