In this study a combined submaximal bicycle exercise test with exercise electrocardiogram, vectocardiogram, echocardiogram and systolic time intervals recorded simultaneously was carried out in 100 normal controls, 37 patients with definite coronary heart disease(CHD)and 48 patients with suspected CHD. Twenty-four variables were analyzed and four of them: Sst-t, △Ds, △mVcf △∑R% were selected to establish the linear function discriminant. The result showed that discriminant analysis raised the sensitivity from 57% judged by ST segment criterion alone to 92% (p<0.001); specificity,from 96% to 100% (P<0.05); predictive accuracy, from 84% to 100% (p<0.02); true negative rate, from 86% to 97% (p<0.01); correct classification rate, from 85% to 98%(p<0.001), The mechanisms responsible for the diagnostic contributions of these new indexes were discussed. We conclude that using multivariable discriminant analysis to combine indexes reflecting mechanical changes of myocardial ischemia with quantification of ST-T changes, the diagnostic value of exercise test to CHD can be significantly increassed. This method is a new diagnostic approach to CHD and deserves further investigation.

运用心电图、心向量图、超声心动图和收缩时间间期四种无创性检查方法,对100例正常对照者、37例冠心病患者和48例可疑冠心病患者进行了联合次极量卧位蹬车运动试验。结果表明运用联合运动试验的多变数判别分析的方法,将无创性检查与运动相结合,将反映心肌缺血机械变化的指标与ST—T改变的定量化相结合,能有效地克服目前心电图运动试验的假阴性和假阳性,提高冠心病诊断的敏感性、特异性和予测准确性,同时可定量判断冠心病患者的病变程度和心功状态。这是冠心病诊断的一条新途径,需要继续研究。

The preliminary results of double 2-step exercise test in 300 cases are reported. At the end of 2.5-22 years' follow-up 46 of the 47 cases with positive excercise tests proved to have coronary heart diseases (97.9%). Among the latter 4 cases had acute myocardial infarction. 45 of the 53 cases with doubtful exercise tests proved to be suffering from coronary heart disease and two of them had acute infarctions. However, among the 200 cases with negative exercise tests 106 eventually had definite coronary heart...

However, among the 200 cases with negative exercise tests 106 eventually had definite coronary heart diseases. These results demonstrated that the predicative accuracy, the sensitivity as well as the specificity of the exercise tests for those who had symptoms were higher than the symptomless patients.

本文报告300例双倍二级梯运动试验经2.5~22年随访结果。初测为阳性的47例中,后证实有冠心病者46例(97.9),其中4例心肌梗塞;可疑阳性53例证实有冠心病45例(84.9%),2例心肌梗塞,阴性组200例确诊冠心病106例(53.0%)、有16例心肌梗塞,内4例在随访中死亡。均经尸检证实。本组结果表明,运动试验对有症状人群的冠心病的预测准确性、敏感性及特异性较无症状者为高。
