Eighteen commercially pure titanium Steri-Oss implant fixtures were placed into the fresh extraction sockets. This study was performed to compare the efficacy of guided tissue regeneration (GTR) around dental implants immediately placed into extraction sockets by resorbable or nonresorbable membranes. Methods Mandibular P2, P3, and P4 of four adult beagle dogs were extracted bilaterally, and buccal standard defects were created and measured. Eighteen commercially pure titanium Steri-Oss implant fixtures were placed into the fresh extraction sockets. Four untreated implants were as controls, four implants received polytetrafluoroethylene (e-PTFE, Gore-Tex) membranes, five implants received collagen membranes, and five implants received polyglactin 910 mesh. After 14 weeks, clinical measurements were taken and the dogs were sacrificed and all specimens retrieved for histologic and histomophometric evaluation. Results The average gain in bone height was 2. 1 mm for untreated control sites, 3. 3 mm for Gore-Tex sites, 3. 8 mm for collagen sites, and 1. 3 mm for polyglactin 910 sites. Conclusion The results of this study indicated that Gore-Tex and collagen membrane produced good results for GTR around implants. Collagen membrane can bea valid alternative to Gore-Tex membrane to improve bone regeneration around dental implants, while polyglactin 91O mesh seemed not suitable to be used as GTR membrane in immediate implantation for its high infection rate.

X-Radiographs were taken at the tenth day, the first month, the second month and the sixth month after implant placement to document the bone fill of peri-implant bony defects and osseointegration of implants. The results were as follows. all implants were clinically osseointegrated and there was bone regeneration in all peri-implant bony defects. PTFEM with and without early explosure during healing has important effect on effectiveness of gueded tissue regeneration in peri-implant bony deffects.Asignificant diffevencewas found in the rate of bone fill between the teams with and without PTFEM early explosure (P<0.01).

为了评价引导组织再生(GTR)技术引导种植体周骨缺隙骨再生的效果,作者选择了25例共27颗不能保留的牙齿,于拔除后即刻种植,采用PTFE(聚四氟乙烯膜)覆盖于种植体和牙槽嵴顶上,游离龈瓣严密缝合。术后6个月将PTFE取出。术中和术后6个月分别测量每个种植体周骨缺隙的最大深度,计算出最大骨充填高度和骨充填率。且分别于术后10天、一个月、二个月、四个月和六个月拍X光片观察骨再生和种植体骨结合情况。结果表明:GTR能引导种植体周骨缺隙的骨再生,所有种植体均有骨性结合,从而使拔牙后即刻种植术获得成功。PTFE有无暴露是影响骨再生的重要因素。

Objective: To investigate the clinical results of immediate implantation using Guided Tissue Regeneration (GTR) and the factors influencing the clinical results. Methods: 25 implants were immediately inserted into the fresh extraction sockets from 25 patients. Polytetrafluoroetheylence (PTFE) membrances were put on the top of the alveolar ridge to cover the extraction sockets, the buccal fullthickness flaps were elevated and sutured to palatal flaps to obtain prime closure. Radiographs were taken at the same day, 10 days, 1 month, 2 months, 3 months, 4 months, 5 months and 6 months after implant placement to observe the bone regeneration of peri-implant bony defects. PTFE mimbrances were take off at the 6 months reenty. A year of follow up was taken after restoration of implants were performed. Results: There was no implant loose or lost. The Peri-implant bony defects were fully filled by new bone in 2 months postoperatively, and the new bone turned completely into mature bone and were well osseointegration with implants in 6 months postoperatively. Ealy membrance exposure during healing can result in significant bone loss of alveloar ridge around implants. Conclusion: There were no significant differences in any clinical parameter between immediate implantation using GTR and healed implantation. The barrier function of the PTFE membrance should remain at least for 2 months. Optimal time of taking off the membrance is in 6 months postoperatively. Early menbrance exposure during healing and treatment for it has significant effect on effectiveness of GTR. [

目的 :评价引导组织再生 (GTR)即刻种植术的临床效果 ,探讨影响其效果的临床因素。方法 :将 2 5颗种植立即植入 2 5位患者的新鲜拔牙窝 ,聚四氟乙烯 (PTFE)膜覆盖于牙槽嵴顶封闭拔牙窝 ,松驰唇颊侧粘骨膜瓣 ,严密缝合。术后当天、10天、1、2、3、4、5和 6个月拍X光片检查种植体周骨缺隙骨再生情况。术后 6个月拆除PTFE膜。完成修复后随访观察一年。结果 :无种植体松动或脱落 ;术后 2个月新生骨充满种植体周缺隙 ;术后 6个月新骨完全成熟 ,并与种植体紧密结合 ;膜早期暴露可导致种植体周牙槽嵴顶骨明显吸收。结论 :GTR即刻种植术能取得与延期种植相同的临床效果 ;膜的屏障作用最少应保持 2个月 ,术后 6个月是拆膜的最佳时间 ;膜早期暴露及其处理对GTR的效果有明显的影响
