Methods: ANP was induced by retrograd pancrcatic duct infusion of 3% taurocholate sodium. All animals received parenteral nutrition through the central vein for 72hrs. Serum Amylase (PNPG 7),6 keto PGF 1α , TXB 2, PGE 2were determined by RIA at 4, 48, 72h; The pancreas was dissected for histological scoring at 72h. Results: prostaglandins were increased...

Objective: To study the effects of intravenous fat emulsion on lipid mediators during early stage of acute necrotizing pancreatitis (ANP) in SD rats. Conclusion: Fat emulsion infused in the early stage of ANP can induce increase of serum anti inflammation prostaglandins.

目的:研究静脉输注脂肪乳剂对大鼠急性坏死性胰腺炎早期脂质介质的影响。方法:逆行胰管注射3%牛磺胆酸钠建立SD大鼠坏死性胰腺炎动物模型,通过颈内静脉营养支持72小时,测定4、48、72小时血清淀粉酶、TXB2、6ketoPGF1α、PGE2,72小时行胰腺组织病理学评分。结果:4小时胰腺炎组大鼠血前列腺素类物质均增高,应用脂肪乳剂后可增加血6ketoPGF1α(糖脂组1569±1173pmol/L;全糖组291±61pmol/L,P<005)、PGE2(脂肪组614±15pmol/L;全糖组287±9pmol/L,P<005)含量,病理学评分全糖组和糖脂组间无差异。结论:坏死性胰腺炎早期应用脂肪乳剂后可促进抗炎效应前列腺素的产生

Objective To study the tolerance of lipid emulsion infusion in patients with severe pancrea titis. Method 33 patients with severe pancreatitis were prospectively studied. Among them, 27 cases with normal basal serum triglyceride (TG) levels were randomly divided into A、B and C groups, the other 6 cases with hypertriglyceridemia were allotted to group D. In group A, each was supplied 800~1000kcal/d by glucose as sole fuel source, both group B and C received glucose and lipid emulsion as dual calories substrates (the fat calories account for 40 per cent of total calories in group B and 50 per cent in group C, respectively). The constitution of caloric substrates in group D depended on the levels of serum TG and glucose. The tolerant ability of lipid emulsion infusion was investigated by the fat clearance test.Results The tolerant abilities were good in group B and C under the condition of 65.0~80.0g/d fat infusion. In group D, the fat clearance tests were positive in five cases with serum TG>4.06mmol/L and fasting serum lactescent, another case was weakly positive with serum TG being 3.14mmol/L and clear fasting serum. Conclusion ① For severe pancreatitis patients with normal serum TG levels, infusion of fat 65.0~80.0g/d is tolerable. ② For those with hypertriglyceridemia, whether to use, or not to use lipid emulsion depends on the abnormal of basal serum TG level and whether fasting lactescent serum is present.

目的探索重症胰腺炎病人对静脉输注脂肪乳剂的耐受性。方法33例重症胰腺炎病人,其中27例基础血甘油三酯(TG)值正常者随机分成A、B、C三组,6例基础血TG值高于正常者列为D组。各组均给予肠外营养:A组以葡萄糖单能源供能,热卡量为800~1000kcal/d;在提供热卡与氮源相同的条件下,B、C组均以双能源供能,其糖脂热卡比分别为6∶4及5∶5;D组肠外营养配方根据血TG、血糖测定值而定。以脂肪乳剂廓清试验观察对输注脂肪乳剂的耐受性。结果B、C组每日输注脂肪65.0~80.0g均能耐受;D组有5例基础血TG值>4.06mmol/L,且空腹血清呈乳状,脂肪乳剂廓清试验阳性,1例基础血TG值为3.14mmol/L,空腹血清不呈乳状,脂肪乳剂廓清试验弱阳性。结论①基础血TG值正常的胰腺炎病人,每日输注占总热卡40%~50%的脂肪乳剂均能耐受;②伴高TG血症的胰腺炎病人,应根据基础血TG值增高的程度及是否存在空腹乳状血清,来判断应用、慎用或不用肪脂乳剂。

目的观察静脉输注脂肪乳剂对伴高甘油三脂(TG)血症胰腺炎大鼠胰腺的病理损害程度。方法SD大鼠静脉滴注蛙皮素(cerulein)诱导水肿性胰腺炎,静脉注射辛基苯基聚氧乙烯醚(TritonWR1339)诱导高TG血症。60只大鼠随机分为5组:A组(胰腺炎组);B1组(轻度高TG血症+胰腺炎组);B2组(轻度高TG血症+胰腺炎+脂肪乳剂组);C1组(中度高TG血症+胰腺炎组);C2组(中度高TG血症+胰腺炎+脂肪乳剂组)。定期测定血清TG值,淀粉酶(AMS)值,测定胰腺湿重,腹水评分以及光镜观察胰腺组织病理损害程度。结果B2组血清TG值、AMS值,胰腺湿重测定,腹水评分及胰腺病理组织学评分与B1组比较差异无统计学意义;C2组血清TG值及AMS值均较C1组明显升高(P<0.05),胰腺湿重测定,腹水评分及胰腺病理组织学评分较C1组加重。结论伴高TG血症的胰腺炎大鼠,当其血清TG值达到一定程度≥(3.86±0.36)mmol/L时,静脉输注脂肪乳剂,将导致血清TG水平进一步升高,胰腺炎病理损害加重。
