The microvasculature of monkey parotid gland was observed by scanning electron microscope. The capillary networks around the acini were loose and the capillary networks around the ducts were dense and sinusoidal in type. The capillary networks around both the acini and intercalated duct and sinusoidal capillary networks around both the striated duct and intralobular duct were supplied by the blood passing through the acinar or duct arterioles from interlobular and intralobular artery. The capillary networks around the acini showed three draining forms: (1) draining into the vein directly; (2) draining into the capillary network around the striated duct through capillaries; (3) draining into the capillary network around the striated duct through venules. The latter form (venules) is named as "portal system". The capillary networks around the striated duct showed two draining forms: (1) they continued to form the capillary network around the intralobular duct; (2) they converged into venules which accompanied by the intralobular duct. The arterio-venous anastomoses were not observed in the parotid gland. However, arterio-arterial and venovenous anastomoses were found in interlobular region.

We found that the arterial blood circuited,direct partly through branches of vein and partly into capillary network through micro vein. Some also flew through micro vein and microartery into capillary nctwork. The hlock of venous volve was likely to cause iscemic necrosis in the distal aspect of the flap and hlood extravasation in the proximal aspect. So thevessels should be anastomosed according to the direction of venous flow. With more available donor sites and no sacrificeof named artery, this method is still applicable in suitable cases despite high occurrence of swelling, congestion, blood ex travasation and prolonged period for quality recovery.

我院应用4例前臂动脉他静脉皮瓣及离体标本的灌注,探讨了此皮瓣的血循环机理。动脉血经静脉支部分直接回流,部分进入微静脉经毛细血管达真毛细管网。另部分动脉血入微静脉经动静脉吻合支返回微动脉入后微动脉再入真毛细血管网。因静脉瓣的阻碍,使引流侧皮瓣远端灌流困难,易造成缺血坏死而灌流侧易成瘀血性坏死。因此理想血供应是动脉化干及引流干均应顺静脉血流方向缝接.减少静脉瓣阻力。动脉化静脉皮瓣易发生肿胀、充血、瘀血,质地须较长时间才能恢复正常。但是,此皮瓣来源多,不须牺牲知名动脉,适当病例可选用。

Objective To investigate the changes of microvessel on the intrahepatic bile duct carcinogenesis in Syrian hamsters.Methods On the base of the animal model induced in Syrian hamsters, the microvessel was observed by combining with histology and vascular cast and scanning electron microscope during the ICC carcinogenesis of Syrian hamsters.Results In the normal intrahepatic bile duct, blood capillary regularily arrayed , formed integrated capillary network, the shape of blood capillary was regular, the courser of blood capillary was natural and the basement membrane was intact. In the hyperplasia bile duct, the quantity of blood capillary slightly increased, blood capillary had basement membrane. In adenoma, the quantity of blood capillary obviously increased, its basement membrane was not intact.In atypical hyperplasia bile buct, the quantity of blood capillary also obviously increased, blood capillary became wrench, its basement membrane became non-intact.In the bile duct adenocarcinoma, the quantity of blood capillary significantly increased and angiectasia, pipe cava became big and dissepiment became frail.The structure of blood capillary was absent from integrity, its basement membrane became gracile or deficient, the arrangement of blood capillary had no regularity,the rich capillary network was formed.Conclusion Changs of microvessel give a condition for intrahepatic bile duct carcingogenesis.

目的探讨在肝内胆管癌发生过程中微血管变化情况。方法本研究在叙利亚地鼠肝内胆管癌动物模型的基础上,在肝内胆管癌形成过程中应用组织学和血管铸型、扫描电子显微镜相结合观察微血管变化情况。结果正常肝内胆管毛细血管排列规律,形成完整的毛细管网,毛细血管外形规则、走行自然,有完整的基底膜。增生胆管血管轻度增多,有基底膜。胆管腺瘤血管明显增多,基底膜不完整。不典型增生毛细管也明显增多,扭曲,基底膜不完整。胆管腺癌毛细血管显著增多,血管扩张,管腔不规则,结构缺乏完整性,管壁薄弱,基底膜变薄或缺如,血管盘曲,无规律性,形成丰富的毛细血管网。结论提示微血管变化为肝内胆管癌的发生提供条件。
