Objective:To s tudy the clinical value of 3D reconstruction of tracheobronchial tree for lar ge bronchi diseases with multislice spiral CT(MSCT)retrospectively.Materials and Methods:Thirty-four patients with large bronchi diseases veri-fied by pathology performed MSCT volume scan.Scan conditions:120kV,200~250 mAs,slice thick1.25~2.5mm,pitch3.The primitive images were transmitt ed to workstation to perform3D reconstruction.Analyzing3D images,including s haded surface display(SSD),minimum intensity projection(MinP),CT virtual b ronchoscopy(CTVB),multiplanar reconstruction(MPR).Result:3D reconstructi on of tracheobronchial tree with MSCT could completely reveal abnormal changes of large bronchi diseases,and exactly define its position,scope,degree and bronchus situation beyond the focus.Among32cases with large bronchi diseases ,diagnosis correct-rate of 3D images is93.75%.Conclusion:3D reconstr uction of tracheobronchial tree with MSCT may play an important role in diagno sis or differential diagnosis,pre-operational assessment and post-operation al reexamination of large bronchi diseases.

目的:回顾性研究多层螺旋CT气管、支气管树三维成像对于大气道病变的临床应用价值。材料与方法:34例有病理证实的大气道病变的病例均经多层螺旋CT(MSCT)容积扫描。扫描条件:120kV,200~250mAs,层厚1.25~2.5mm,螺距为3。轴位图像传入工作站后进行三维重建。分析表面遮盖法(SSD),最小密度重建(MinP),仿真内窥镜法(CTVB),多平面重建(MPR)等多种重建技术的影像表现。结果:MSCT气管、支气管树三维成像可以直观、完整显示段支气管以上不同大气道病变的异常形态学改变,明确大气道病变的位置、范围、程度以及病变以远侧支气管情况。32例支气管疾病中,三维图像诊断准确率为93.75%。结论:MSCT气管、支气管树三维重建对于大气道病变的诊断与鉴别诊断、术前评价以及术后复查均具有重要价值。
