Objective:To explore the reliability of hydroxylation phenotype got by a concentration ratio of risperidone to 9-hydroxyrisperidone,and to find their correlations with adverse reactions.Methods:40 health Chinese volunteers with a single oral dosage of risperidone were included in this study.LC-MS method was used to determine the concentrations of risperidone and 9-hydroxyrisperidone in human plasma,the risperidone plus 9-hydroxyrisperidone and the ratio of risperidone to 9-hydroxyrisperidone were calculated,the scatter plots were established and the cutoff used to phenotype hydroxylation distribution were selected.Results:Risperidone metabolism was polymorphic,and fourty volunteers could be divided into extensive(EMS),intermediate(IMS)and poor metabolizers(PMS)by the ratio of risperidone to 9-hydroxyrisperidone.The poor metabolizers would be susceptible to risperidone side efects.Concolusion:The ratio of risperidone and 9-hydroxyrisperidone can be used to phenotype hydroxylation distribution.The total concentrations of risperidone and 9-hydroxyrisperidone can be used to modify risperidone dosage regimen.

目的:探讨利培酮与9-羟利培酮血浓度比值确定人体羟基化代谢表型的可行性以及利培酮、9-羟利培酮、利培酮+9-羟利培酮、利培酮/9-羟利培酮与羟基化代谢表型及不良反应间的关系。方法:采用液质联用技术测定单剂口服利培酮后利培酮及9-羟利培酮血浓度,计算利培酮+9-羟利培酮、利培酮/9-羟利培酮值,绘制利培酮与9-羟利培酮血浓度比值累积频数图,选择快慢羟基化代谢表型的截点。结果:根据利培酮与9-羟利培酮血浓度比值,40名志愿者可分为快代谢(EMS)、中代谢(IMS)与慢代谢(PMS)个体;EMS与IMS相比,利培酮和9-羟利培酮血浓度有统计学意义(P<0.05),而利培酮+9-羟利培酮无统计学意义。PMs与EMS组或IMs个体间利培酮、9-羟利培酮、利培酮/9-羟利培酮和利培酮+9-羟利培酮差异均无统计学意义。PMS个体更易发生锥体外系反应。结论:利培酮可用于羟基化代谢表型测定;监测利培酮与9-羟利培酮血浓度有助于修饰利培酮治疗方案。
