Objective To determine if the EMG changes of sternocleidomastoid muscle are relevant to the bulbar palsy and helpful in the assessment of subclinical bulbar involvement, and elucidate the value of EMG study of sternocleidomastoid muscle in the diagnosis of amyotrophic lateral sclerosis Methods Standard EMG was recorded from at least three limbs (one side abductor pollicis brevis muscle and two sides tibialis anterior muscle) and one side sternocleidomastoid muscle of 40 ALS patients with clinical bulbar signs and 50 ALS patients without clinical bulbar signs Parameters studied included spontaneous activity, duration, amplitude and percentage of polyphasic wave of motor unit potential (MUP); pattern of recruitment EMG parameters of sternocleidomastoid muscle were compared between patients with and without bulbar palsy; the differences of EMG changes among sternocleidomastoid muscle, upper limbs and lower limbs were also analyzed Results EMG analysis of sternocleidomastoid muscle showed neuropathic changes in all 90 ALS patients, the increase of MUP duration was ranged from -8%~104%,mean 54 7%±20 3%; the increase of MUP amplitude was ranged from -20%~270%; spontaneous activity was only found in 3 cases; decreased recruitment was found in 20 cases (22 2%), and the increase of MUP duration and pattern of recruitment were significantly different between patients with and without bulbar palsy ( P <0 05) The increase of MUP duration and amplitude, spontaneous activity and pattern of recruitment were significantly different between sternocleidomastoid muscle and limbs; while there was no difference between upper and lower limbs in spontaneous activity, pattern of recruitment and the increase of MUP duration If the EMG neuropathic changes of sternocleidomastoid muscle were attributed to bulbar part but not upper limb, the number of patients diagnosed as definite and probable ALS was increased significantly in ALS patients without clinical bulbar signs ( P <0 05) Conclusion The EMG neuropathic changes of sternocleidomastoid muscle is related with the symptoms of bulbar part but not cervical part and should be regarded as an evidence of lower motor neuron involved in bulbar part

目的 探讨胸锁乳突肌肌电图 (EMG)检测在肌萎缩侧索硬化症 (ALS)诊断中的价值。方法 对ALS患者均进行常规EMG检测 ,包括上肢、下肢及胸锁乳突肌 ,测定自发电位 ,募集相 ,运动单位电位的时限、波幅及多相波。比较有、无球部症状的患者之间胸锁乳突肌EMG改变的差异 ,以及不同部位即球部、颈部及腰骶部之间EMG检测结果的差异。结果 有球部症状的患者胸锁乳突肌EMG病变程度重于无球部症状者 (时限增宽幅度分别为 60 6 %± 2 1 2 %和 50 0 %± 1 9 2 % ,P <0 0 5)。胸锁乳突肌EMG自发电位、募集相、运动单位电位时限和波幅改变的程度均不同于上肢和下肢 ,存在显著性差异。结论 胸锁乳突肌EMG检测的异常与球部症状相关 ,而与肢体无关 ,其神经源性改变可反映球部下运动神经元病变

Methods The fib and psw of thoracic paraspinal muscles,the motor unit potential of sternocleidomastoid muscle were detected in 50 patients who had been identified as having ALS.Age and sex-matched,30 normal subjects and 30 patients who suffered radicular disease in cervical or lumbosacral region were also detected in thoracic paraspinal muscles for finding fib and psw serving as controls. Results In ALS group,a vast of fib and psw were found in thoracic paraspinal muscles in 41 of 50 ALS patients (82%),they were not found with sternocleidomastoid muscle,but the motor unit potential of this muscle was shown having long-duration and high-amplitude in 48 of 50 ALS patients (96%),which were coincided with the chronic neurogenic changes.6 of 50 patients had denervation changes of tongue muscles,3 of them found fib and psw.2 of 30 patients who suffered from radicular diseases were found with a few of fib and psw changes in thoracic paraspinal muscles.None of these symptoms were found in normal subjects.Conclusion A vast of fib and psw in thoracic paraspinal muscles should represent a sensitive tool for diagnosis of ALS.

目的 研究胸段脊旁肌自发电位在诊断肌萎缩侧索硬化 (amyotrophiclateralsclerosis,ALS)中的价值。方法  5 0例确诊的ALS患者分别行胸段脊旁肌自发电位、胸锁乳突肌运动单位 ,部分患者行舌肌自发电位检测 ;同时 ,将性别、年龄相匹配的 30例根性损害的患者和 30名健康人作为对照 ,行胸段脊旁肌自发电位检测。结果  5 0例ALS患者中 ,4 1例 (82 % )胸段脊旁肌肌电图可见大量纤颤电位和正锐波 ;胸锁乳突肌肌电图无一例见自发电位 ,但有 4 8例 (96 % )运动单位时限增宽 ,波幅增高 ,符合慢性神经源性损害改变 ;6例患者行舌肌自发电位检测 ,3例可见自发电位。 30例根性病变的患者中 ,2例 (7% )可见胸段脊旁肌有少量自发电位 ;健康对照组未见胸段脊旁肌自发电位。结论 胸段脊旁肌大量自发电位对诊断ALS具有一定的敏感性。

The motor unit potentials recorded from the sternocleidomastoid muscles in 94 patients of group 2 (92.1%) were with longer duration and higher amplitude. Few spontaneous potentials were found in 6 of 96 patients with radiculopathy (0.2%) . No abnormality manifes tations were found in patients of group 3. Conclusion EMG exam of the lower thoracic paraspinal muscles is helpful to the diagnosis of MND.

目的 探讨下胸段脊旁肌肌电图在诊断运动神经元病 (MND)中的价值。方法 共检查 3组受试者 ,即A组为 10 2例确诊的MND患者行常规上下肢肌肉、下胸段脊旁肌和胸锁乳突肌肌电图检测 ;B组为性别、年龄相匹配的 96例颈腰脊神经根损害患者 ,行常规上下肢肌肉、下胸段脊旁肌肌电图检测 ;C组为性别、年龄相匹配的 10 0名健康人 ,行常规上下肢肌肉、下胸段脊旁肌肌电图检测。结果  10 2例MND患者中 ,87例 (85.3 % )下胸段脊旁肌肌电图可见大量纤颤电位和正锐波 ,94例 (92 .1% )胸锁乳突肌肌电图运动单位时限增宽、波幅增高。 96例颈腰脊神经根损害的患者中 ,6例 (6.2 % )下胸段脊旁肌肌电图可见少量自发电位。 10 0名健康人下胸段脊旁肌肌电图未见异常。结论 下胸段脊旁肌肌电图有助于MND的诊断
