The results showed that T-cell functions, phagocytosis of leukocytes Fn level were decreased markedly and CRP as well as serum C5a activity were much higher in the MOFE than those of normal control group (P<0.01).Further more, all these changes corresponded well with the severity of the MOFE. This study implies that the immunodeficiency may play an important role in the development of MOFE. The functional state for host immune system is closely related to the prognosis of MOFE. Monitoring immune function could be a significantly index for determination of prognosis.

    对41例老年多器官功能衰竭(MOFE)患者进行了多项免疫功能指标监侧,并与健康组进行比较。结果表明:MOFE组的T细胞功能、白细胞吞噬活性、血浆纤维结合素明显降低(P<0.01),C-反应蛋白、过敏毒素活性明显增高(P<0.01),且随病情的好转、恶化而发生明显的变化。MOFE死亡组部分患者病情恶化时的免疫功能指标变化与存活组病情好转时比较,差异有显著性(P<0.01)。提示免疫功能低下和紊乱是老年人易发生感染、导致MOFE的主要因素,而且免疫功能的损伤程度与MOFE病情严重程度密切相关。因此认为,上述免疫功能指标可做为早期判断MOFE病情及预后的有意义的预警指标。

The sequence of failured organ was lungs, heart,brain, kidneys and liver. The mortality of the patients with failure of 2-4 organs was 51. 9%-83. 3% and 5 organs was 100%.     分析116例老年人以感染为主要诱发因素的多器官衰竭(MOF)的临床资料,发现由肺部感染所诱发的MOF居首位,占81.0%,细菌学检查以革兰氏阴性杆菌居多。多器官衰竭的先后顺序为:肺、心、脑、肾、肝。2~4个器官衰竭者病死率为51.9%~83.3%,5个以上器官衰竭者13例,全部死亡,病死率100%。提示老年人各器官功能不全,免疫功能低下是MOF的主要因素,肺部感染为始动因素。并对其预后及诊治中的经验教训进行了讨论。
