In order to evaluate combined serum tumor enzyme determinations in the diagnosis of hepatocellular carcinoma (HCC), four tumor markers including gamma-glutamyl tran- speptidase isoenzyme (r-GT-II), alkaline phosphatese isoenzyme (ALP-I), the ratio of the two aldolase activities, fructose-1, 6-diphosphoaldolase and fructose-1-phosphoaldo-lase (FDP/FIP) and a-L-fucosidase (AFU) were concomitantly determined in 59 patients with HCC, 97 patients with various liver diseases and gastrointestinal tumors. The results revealed that: (l)r-GT-II, ALP-I, FDP/FIP and AFU level were increased significantly in HCC (P<0.01); (2) The diagnostic sensitivity and specificity of these markers in HCC were r-GT-II(79.7%)>AFU(78.8%)>FDP/FIP(61%)>ALP-I(16.95%) and ALP-I(100%) >r-GT-II(92.l%)>AFU(90.9%)>FDP/FIP (88.8%), respectively; (3) The diagnostic positivity of combination assay was 80.0% in low or negative AFP producing HCC, while 95.7% in those with elevated AFP. Our data suggested that the combination assay of serum tumor enzyme markers was valuable in the diagnosis of HCC, especially in low or negative AFP HCC patients

本文对59例原发性肝癌(HCC)和97例其他肝病及消化道肿瘤患者进行了血清肿瘤酶标志物的联合测定。结果显示HCC组的r-谷氨酰转肽酶同工酶、碱性磷酸酶同工酶、醛缩酶同工酶及血清α-L-岩藻糖苷酶的活力均较正常组显著增高(P<0.01),四种酶联合检测对AFP(一)HCC的诊断阳性率为80%,与AFP联合检测对HCC总的诊断检出率达95.7%

The diagnostic significance of serum α-L-Fucosidase (AFU)in patients with primary hepatic carcinoma(PHC, 36 cases), liver cirrhosis (LC, 30 cases), metastatic liver carcinoma (MC, 16 cases), benign space-occupying lesion of liver (BSOL, 11 cases), chronic hepatitis (CH, 22 cases) and enterogastric malignant tumors (EMT, 20 cases), was investigated. The serum AFU activity was measured by spectrophotometry. The AFU, αianti-trypsin (α_1AT) and α-fetoprotein (AFP) were compared between the patients with liver occupying lesions. The results showed that serum AFU activity in patients with PHC was higher than that in any other groups. The sensitivity of AFU in the diagnosis of the patients with PHC was higher than that of AFP and α_1AT, which were 75%, 61.1% and 66.7% respectively, and the reliability of AFU, AFP and a_1AT were 81.7%, 83.9% and 65.6% respectively. Combining the three markers, the rate of diagnosis in patients with PHC was raised to 91.7%. There was no correlation between the tumor size and AFU activity in patients with PHC, The data denoted that serum AFU was considered as one of the serum markers in patients with PHC, especially in cases whenever were negative.

用分光比色法测定血清α-L-岩藻糖苷酶(AFU),研究其对36例原发性肝癌(PHC)、30例肝硬化(LC)、16例转移性肝癌(MC)、11例良性肝脏占位性病变(BSOL)、22例慢性肝炎(CH)和20例胃肠道恶性肿瘤(EMT)患者的临床应用价值。在肝脏占位性病变组中对AFU,AFP和α_1AT进行比较研究。结果表明:PHC组血清AFU活力显著高于其余各组,AFU,AFP和α_1AT对PHC诊断敏感性分别为75%,61.1%和66.7%,可靠性分别为81.7%,83.9%和65.6%,三项指标联测则PHC诊断阳性率可提高到91.7%。AFU活力与PHC肿瘤大小、AFP含量无相关,与α_1AT呈正相关。本文资料表明:AFU可作为PHC血清标志物,对AFP阴性病例尤有价值。

By means of chromogenic peptide substrate method,serum α-L-fucosidase

本文用显色多肽基质法检测血清α-L-岩藻糖苷酶(AFU)共167例,结果进一步证明AFU 检测对原发性肝癌(PHC)(72例)诊断敏感性及特异性分别达75. 0%及90. 5%;在 AFP 阴性或低滴度阳性 PHC 病例中的阳性率达83. 3%;AFU 与AFP 联合检测使诊断 PHC 的阳性率上升为93. 1%。且随病情好转和恶化而下降和上升。血清AFU 值 EdmondsonⅡ级肝癌明显高于Ⅲ级。PHC 肿瘤组织 AFU 含量明显低于非肿瘤组织,提示 AFU 为分泌型肝癌标志物。
