【目的】探讨采用大剂量甲氨蝶呤 (MTX)预防性治疗中枢神经系统白血病 (CNSL)对急性淋巴细胞白血病(ALL)儿童智力与行为的影响。【方法】①按“中国 比内智力测验手册”方法对 4 2例ALL患儿在治疗前后与 4 0例正常儿童进行测试比较 ;②根据自制行为量表对ALL患儿治疗前后进行行为比较。【结果】①ALL患儿治疗前IQ、行为与对照组无差异 (P >0 0 5 ) ,而治疗后IQ明显低于对照组 ,(P <0 0 1) ;加用60 Co颅脑放疗组IQ降低更为明显。②ALL患儿治疗后行为问题出现率明显高于对照组 (P <0 0 1)。【结论】大剂量MTX治疗对ALL儿童的智力与行为有不同程度的损害作用 ,加用60 Co颅脑放疗影响更为明显

Objective To explore the relationship between lead level and children's intelligence and behavior. Methods 62 children (6~12 years old) from psychological consulting department of hospital were enrolled in the study. The Wechsler Intelligence Scales for Children(Chinese Version 1993) and Achenbach Child Behavior Checklist (CBCL) were applied to measure children's intelligence quotient(IQ) and behavior problems. The concentration of hair and serum lead were examined. Pearson Correlation Analysis and Generalized Linear Model were adopted to test the relationship between the lead level and children's IQ and behavior problems. Results The negative association was found between hair lead concentration and children's IQ (r=-0.264,P< 0.05 ). High level of hair lead concentration would do harm to intelligence. Conclusions Hair lead concentration is sensitive to measure children's lead exposure. It is valuable to scan the vulnerable children from those who suffered from lead exposure.

目的 研究学习困难儿童铅水平对其智力与行为的影响。方法 采用中国修订版韦氏儿童智力量表 (C- WISC)和 Achenbach儿童行为量表 (CBCL )对 6 2例 6~ 12 a因学习问题来心理门诊就诊的儿童进行了测评 ,同时测试了发铅、血清铅水平 ,对两组指标进行相关分析。为控制混杂因子 ,应用广义线性模型 (GL M)分析了发铅、血铅对儿童智商的作用。结果 发铅水平与儿童智商呈负相关 (r =- 0 .2 6 4 ,P <0 .0 5 ) ,GL M分析结果表明高发铅可能对儿童智力有影响。结论 发铅是反映儿童铅暴露的较好指标 ,因检测相对简便、经济 ,对筛查儿童高铅人群有一定价值
