Multiple myeloma (MM) is a neoplasm characterized by accumulated malignant plasma cells which can secret a large mount of monoclonal immunoglobulin in bone marrow.

Waldenstrm′s macroglobulinemia (WM) is one of malignant hematological disease on account of abnormal proliferation of B lymphocyte clone and the pathologic cells of WM possess ability to secrete monoclonal immunoglobulin M. In this study, the diagnosis and morphological characteristics of 2 patients with WM were analyzed.

Results In 798 cases with positive M protein, there were 648 cases of MM (81 2%) and 34 cases of Waldenstrom′s maeroglobulinemia (WM) (4 3%). The remaining 116 cases (14 5%) were benign or secondary monoclonal gammopathies.
The average M protein content of 116 cases of benign or secondary monoclonal gammopathies was (11 3±6 5) g/L. 55 of the 116 cases were monoclonal gammopathy of undetermined significance (MGUS), which accounted for 6 9% of all the cases.

The IgM pentamer in the sera of Waldenstrm macroglobulinemia patient was reduced by 2-mercaptoethanol to monomer in condition of degasing and the presence of nitrogen, then the sera was applied to IEP to determine the property Of suspected protein. This method is effective for the investigation of IgM monoclonai gammovathv.
Several methods can be used to reduce the IgM pentamer to a monomer with 2-mercaptoethanol in the serum of a patient with macroglobulinemia. Hudson's method is the most effective and is carried out by deoxygenation in the presence of nitrogen. This paper reports the dissociation of serum by different methods in a typical patient and compares the results with immunoelectrophoresis. The degree of success is also discussed.
