本文观察了兔失血性休克及应用小剂量7.5%HS(4ml/kg)复苏时RBC内ATP和2,2.3—DPG水平的变化。20只成年健康家兔被随机分为正常血容量输注HS组、休克输注NS复苏组、休克输注HS复苏组三组。结果显示休克后RBC内ATP和2,3—DPG水平显著下降,应用小剂量HS复苏后两项指标均有所回升,在输注后120分钟有显著增高。由于RBC内ATP和2,3—DPG水平是反映RBC能量代谢的主要指标,而RBC代谢状态又可通过影响RBC诸多功能影响组织的氧代谢和休克的转归。因此,笔者认为应用小剂量HS复苏失血性休克的同时RBC能量代谢状态也发生明显改善,这可能是促进HS有效复苏休克的重要因素。

Besides fluid replacement, the group C underwent escharectomy promptly and grafted with normal murine alloskin.Results\ All parameters decreased remarkably after burn in group A, while the changes in the parameters in group C were similar to those in group B except left ventricular end diastolic pressure (LVEDP). However, at 24 hours after burn, the myocardial contractile function in group B was much lower than that in group C. Conclusion\ Prompt escharectomy after 30% TBSA Ⅲ degree burn is feasible and probably beneficial.

目的研究伤后立即切痂对大鼠心肌力学特别是左心室舒缩功能的影响。方法采用大鼠30%TBSAⅢ度烧伤模型,随机分为烧伤组、烧伤补液组和切痂组。切痂组伤后按Parkland公式补液并立即切痂植以正常大鼠皮肤。颈动脉插管至左心室测量心肌力学参数。结果烧伤组心肌力学参数下降非常显著,烧伤补液组各点参数大多降低,但明显高于烧伤组,除左心室舒张末压(LVEDP)外,切痂组心肌力学参数与烧伤补液组相近。伤后24小时烧伤补液组心肌收缩功能显著低于切痂组。结论在有效复苏下,30%Ⅲ度烧伤大鼠伤后立即实施切痂是可行且有益的
