采用改良的大鼠同种异体心肺联合移植手术,研究了大鼠树突状细胞(DCs)单克隆抗体单独和联合雷公藤多甙片对移植排斥反应的影响,并与生理盐水对照组、雷公藤多甙片(TⅡ)组进行比较。结果表明:大鼠DCs单抗单独和联合TⅡ两治疗组间无明显区别,但两组大鼠移植心脏存活时间明显长于生理盐水组和TⅡ组(P<001);而且病理切片显示以慢性排斥反应改变为主。而生理盐水对照组和TⅡ组皆表现为急性排斥反应。提示DC单抗在治疗移植排斥反应中有实用价值。

Objective To investigate the effect of T cell vaccination (TCV) on the survival time of cardiac allograft in rat. Methods Rat heterotopic cardiac transplantation was performed. Donor antigen-primed recipients' spleen cells were obtained by 1/3 splenectomy for the preparation of TCV. TCV was then intraperitoneally into the abdominal cavity again. The survival time of cardiac allograft was observed. Results TCV could markedly prolonged rat cardiac allograft survival. T cell proliferation was increased and B cell proliferation was not affected in the vaccinated rats. The mixed lymphocyte reaction (MLR) was inhibited. The analysis of phenotypes on T cells showed that the number of CD8 + clones in vaccine cells and vaccinated rat spleen cells were increased. The antibody-mediated cytotoxicity was not affected by TCV. Conclusions TCV enhances the recipients' T cell-mediated immune response. The TCV-induced hyporeactivity to specific donor antigen is related to the induction of anti-idiotypic response and is not associated with humoral mechanisms.

目的 探讨T淋巴细胞疫苗 (TCV)对大鼠移植心脏存活时间的影响及其机理。方法 取经供者抗原致敏的受者脾细胞 (1/3脾切除 )制备TCV ,然后将其回注于自身腹腔 ,观察大鼠异位移植心脏的存活时间。结果 接种TCV可明显延长大鼠同种移植心的存活时间 ;注射TCV后 ,T淋巴细胞增殖反应增强 ,B淋巴细胞增殖反应未受影响 ;混合淋巴细胞反应 (MLR)降低 ;脾细胞及TCV表型分析显示CD8+细胞增多 ;抗体介导的补体依赖性细胞毒作用无明显变化。结论 TCV接种是一个免疫刺激过程 ,其所介导的免疫低反应具有抗原特异性 ,可能与抗独特型反应增强有关 ;T淋巴细胞疫苗延长移植物存活时间的机制可能与体液因素无关。

Objectiveo study immune tolerance induced by blockade of B 7 CD 28 interaction after heart transplantation in rats. Methods[WT5”BZ] A model of rat heterotopic heart allograft was established by using Ono′s technique. CTLA 4Ig was administered by intraperitoneal injection at dose of 100 μg. Survival times, histological changes, and the concentration of IL 2, IgG and IgM were measured after operation. Results[Survival times were 31 60±1 82 days in experimertal group and vs 7 25±0 71 days in control groups. In the experimental group, the graft survival time prolonged significantly ( P <0 01). Histological changes were characterized by mild scattered mononuclear infiltration and patchy myocyte injury in the experimental group (grade Ⅰ Ⅱ). In the control group, the changes were characterized by severe lymphocyte infiltration, myocyte injury and necrosis, interstitial edema, and hemorrhage (grade Ⅳ). IL 2 was significantly decreased after operation ( P <0 01). IgG and IgM were not changed significantly in comparison with those in the control group ( P >0 05). Conclusion The blockade B 7 CD 28 interaction can induce rat heterotopic heart allograft immune tolerance. It may provide a new way to treat immune rejection after transplantation.

目的 研究阻断B7方法诱导大鼠心脏异位移植的免疫耐受。 方法 采用Ono法建立大鼠异位心脏移植模型 ,实验组腹腔注射B7阻断剂CTLA 4Ig,观察大鼠移植的心脏存活天数 ,病理改变和术后IL 2 ,IgG以及IgM含量的变化。 结果 实验组移植的心脏存活时间为 :(31 6 0± 1 82 )d ,对照组 (7 2 5± 0 71)d ,差异有显著性意义 (P <0 0 1)。组织学改变 :实验组见局灶性血管周围或间质内淋巴细胞浸润和局灶性心肌损伤 ,病理分级Ⅰ~Ⅱ级。对照组可见大量淋巴细胞浸润 ,心肌细胞损伤和坏死 ,间质内出血水肿 ,病理分级Ⅳ级。术后IL 2含量实验组比对照组明显下降 ,有显著性差异 ,P <0 0 1。IgG和IgM含量差异无显著性意义 (P >0 0 5 )。 结论 阻断B7能诱导大鼠心脏移植免疫耐受 ,为移植排斥反应提供了新的治疗方法
