Serum and urine osmotic pressure (SOP & UOP), urine/blood osmotic ratio (OR) and free water clearance (CH2O)were studied in 80 cases suffering from renal diseases n conjunction with their clinical manifestations and renal functions.It was noted that the higher the SOP values, the more the retention of BUN and the lower the endogenous creatinine clearance. SOP values in 10 uremic cases were all above 300 mOsm/KgH2O. Four cases of azotemia with normal SOP values had hyponatremia. Therefore retention of BUN could not be ruled out by the normal values of SOP.In accordance with the fact that the lower the endogenous creatinine clearance, the smaller the value of UOP and OR, CH2O was shifted from negative to positive value. Among 8 cases of normal endogenous creatinine clearance with obvious declining UOP, renal tubular or medullary damages were as a rule present.Urine osmolarity was quite closely correlated with urine specific gravety. The latter, interfered by many factors, could not accurately reflect the concentrating function of kidney. Because the result of 2-hour PSP test varied with the exact amount of dye injected, the desired volume of urine collected and the renal blood flow uncontrolled, so it was not so accurate and convenient as UOP determination.

应用渗透压计测定血、尿等各种体液的渗透浓度以研究水、盐代谢紊乱,国外已作为临床常规检验方法。肾脏是调节水、电解质、尿素氮的重要器官,因此,血及尿渗透压测定也是检查肾功能的重要方法之一。 本文报道应用国产FM型冰点渗透压计测定80例慢性肾脏病人的血清及尿渗透压、尿血渗比和无溶质水清除率,并结合其临床表现及其肾功能指标进行分析。

The Ser Was raised in none of the cases, whilethe CH_2O increased in 29.2% of them, but the U/P<1 was shown in 10.4%the FEMg increased in association with a drop of TRMg occured in 85.0% ofthem.It is concluded that both reanl glomerular filtration and tubular functionwere impaired in variable degrees in these patients during respiratory failure.The causes and effects of these abnormalitis of reral functional changes werediscussed.

为了探讨慢性呼吸衰竭急性期肾功能状态,本文对66例慢性肺心病患者进行了肾小球滤过功能(BUN、Scr、Ccr)及肾小管功能(U/P、C_(H_2O)及FEMg、TRMg)检查。结果:肾小球滤过功能Ccr多减低(62.5%),但BUN多正常(88.9%)而Scr无升高者。肾小管浓缩功能障碍—C_(H_2O)增多者占29.2%,尿/血渗比(U/P)≤1者仅占10.4%。肾小管再吸收功能亦多受累,表现为FEMg增加或TRMg减少者占85.0%。本文尚对呼吸衰竭时肾功能障碍的原因及其影响进行了讨论。
