In1997,the author published a paper"Study on Chinese strategy for Kacshin-Beck disease control",there after our research group began to collect relative new data,information and facts to verify a consideration whether the"strategy"can be with stand temporal try,but no opposition exemplification had been found out yet.Because of the importance of this problem and because of the"strategy"had adopted by government and incorporated into the disease control program for west region of our country,the author think it is a need to explain more deeply on this problem with some new data.The new data mainly is the results of chemical analysis for T-2toxin in cereal samples and in which a fact,catch our eye,is the new data significantly in consistence with the data published by us in1994.This fact,therefore,deepened our understanding about the entirety and persistence of the"strategy".The natural condition and soil in KBD endemic area are suitable for the growth of toxic Fusarium and the cereals produced in sick local become the media and carrier for the propagation and toxin production of these fungus.In the KBD endemic area the ability to propagate the disease for different cereals is in vary different extent,the wheat and maize is strange but rice does have this effect at all.The harvestry,storage and milling of cereals in no sanitation conditions,are the promoting factors for the production and accumulation of T-2toxin in cereals.The theory basis of the strategy is the causes epidemiology and the basic fact is the propagation mechanism of T-2toxin in nature circumstance and life style of the people in local area,therefore the strategy is a systematic protocol,and the core point is to decrease the T-2contents in cereals to a safety level,according to the local conditions.Of this"strategy",based on a right foundation,the problems about KBD control were resorted well and by this program the disease and the sick area can be vanished at all.

Objective To evaluate the value of the change of the blood T-β-HCG concentration in the prognosis of drug abortion in early pregnancy. Methods 589 cases were retrospectively analyzed. Serum T-β-HCG concentration was detected at different time in the women who had received drug abortion to stop the pregnancy. The Receiver Operator Characteristic Curve was used to evaluate the predictive value of the change of the T-β-HCG concentration. Objective To evaluate the value of the change of the blood T-β-HCG concentration in the prognosis of drug abortion in early pregnancy. Methods 589 cases were retrospectively analyzed. Serum T-β-HCG concentration was detected at different time in the women who had received drug abortion to stop the pregnancy. The Receiver Operator Characteristic Curve was used to evaluate the predictive value of the change of the T-β-HCG concentration. Results There was statistical significance in the serum T-β-HCG concentrations and the difference rate detected one week and two weeks after drug therapy between cases of complete abortion and non-complete abortion (P<0.001). The diagnostic sensitivity was 91.5% and the diagnostic specificity was 96.4% when the cutoff value of the T-β-HCG concentration one week after drug therapy was 45.2 mIU/ml. The sensitivity was 87.6% and the specificity was 93.9% when the cutoff value of the T-β-HCG difference rate one week after drug therapy was 0.065. The sensitivity was 93.5% and the specificity was 95.4% when the cutoff value of the T-β-HCG concentration two weeks after drug therapy was 9.2 mIU/ml. The sensitivity was 88.6% and the specificity was 95.9% when the threshold of the T-β-HCG difference rate two weeks after drug therapy was 0.007. Conclusion The change of the serum T-β-HCG concentration in the early pregnant women receiving drug abortion can be used to evaluate the prognosis of drug abortion. The cutoff value are applicable to clinical diagnosis.

目的:检测血T鄄β鄄HCG变化在评估早孕应用药物终止妊娠预后中的应用价值。方法:回顾性分析589例应用药物终止妊娠早孕个体不同时段内T鄄β鄄HCG结果,利用ROC曲线,评价T鄄β鄄HCG数值的改变在评估应用药物终止妊娠是否成功的预示价值。结果:应用药物终止妊娠1周及2周后的T鄄β鄄HCG数值与其改变率在终止妊娠完全和不完全个体中差异有显著性(P<0.001),当1周后的T鄄β鄄HCG临床判断的临界值为45.2mIU/ml时,诊断灵敏度为91.5%,诊断特异度为96.4%,当1周后的T鄄β鄄HCG改变率的临床判断的临界值为0.065时,诊断灵敏度为87.6%,诊断特异度为93.9%。当2周后的T鄄β鄄HCG临床判断的临界值为9.2mIU/ml时,诊断灵敏度为93.5%,诊断特异度为95.4%,当2周后的T鄄β鄄HCG改变率的临床判断的临界值为0.007时,诊断灵敏度为88.6%,特异度为95.9%。结论:T鄄β鄄HCG改变可评估应用药物终止妊娠的预后,所得临床判断限可应用于临床诊断。
