Objective: To report the result of early functional exercises and short operative time by using the external fixation supporter of adjustment for microsurgical operation in the bone and joint of limbs Methods: The defect of bone, skin and vessel in the bone and joint of limbs was treated by microsurgical method and the bone and joint injury was repaired by the external fixation supporter of odjustment cross joint with three Kirschner wires, keeping the free motion of every joint Results: The eighteen cases suffering from defects of bone and joint in hands(11 cases), incomplete amputated lower limbs about the joints (4 cases) and the soft tissue and bone defect of the proximal tibia carrying osteomyelitis (3 cases) were treated, the repaired bone and joint showed normal position and bone union without narrowing of joint space by the X ray films, the average range of motion of the joints was almost normal after the operation for 1 to 3 months All transplanted tissues survived, the extremital limbs of repaired vessels had excellent blood supply Conclusions: Repairring the defcet of bone, skin and vessel in the bone and joint of limbs by microsurgical operation with this method was able to obtain a little injury, stable fix for fracture and exercising early after the operation and promote healing of fracture, and it was able to be easy and quick to operation and be easy to adjust fracture and nurse

目的:报道在四肢骨关节部位行显微外科治疗,应用可调外固定器固定使关节早期活动、简化手术的临床效果。方法:在四肢关节部位有骨缺损或骨折伴有血管损伤、皮肤缺损等病例,应用带血管的骨关节块移植、游离血管移植或各种皮瓣移植等显微外科方法修复后的同时,用三针可调外固定器跨关节外固定治疗骨关节损伤,保持各关节的自由活动。结果:治疗手部骨关节缺损11例;下肢关节部位的不完全离断4例;胫骨近端慢性骨髓炎合并骨和皮肤缺损3例。修复后的骨关节对位对线好,关节间隙存在,术后1~3个月关节活动接近正常,皮瓣全部成活,修复血管后的肢端血运好。结论:该方法在四肢骨关节和血管损伤伴有皮肤缺损的显微外科修复中具有简化手术、损伤小、骨折固定可靠、便于早期关节功能锻炼、方便护理等优点

Objective To ascertain the effect of increasing traction on articular cartilage. Methods Twenty five immature rabbits were randomised into three groups. A miniature Ilizarov external fixator was applied to the right tibia of each rabbit followed by tibial osteotomy. Each group was subjected to different degree of traction lengthening. Objective To ascertain the effect of increasing traction on articular cartilage. Methods Twenty five immature rabbits were randomised into three groups. A miniature Ilizarov external fixator was applied to the right tibia of each rabbit followed by tibial osteotomy. Each group was subjected to different degree of traction lengthening. At the end of lengthening, both knee joints of the animals were dissected out. Sagittal sections of medial and lateral condylar areas were stained with Haematoxylin and Eosin, sanfranin O and Mallory. Results With 15% lengthening (6 rabbits), hypercellularity and vacuole degeneration were detected. With 25% lengthening (7 rabbits), transitional zone cartilage clefts (n=3) and chondrocyte necrosis in the superficial and transitional zone (n=2) were detected. With 35% lengthening (n=9), partial to full thickness cartilage loss (n=4) and degeneration and necrosis of chondrocytes (n=8) were detected. There was slight, moderate and severe reduction of sanfranin O staining in 15%, 25% and 35% groups respectively. Mallory staining showed loss of normal collagen arrangement and collagen thickening in 25% lengthening group and destruction of collagen in 35% lengthening group. Conclusions The “pressure necrosis” occurs during experimental limb lengthening. Its severity is proportional to the degree of lengthening.

目的 了解随肢体延长幅度的增加 ,由受牵拉的跨关节软组织增加的张力产生的关节面之间的压力对关节软骨的影响。方法 对 3组家兔分别进行不同幅度的胫骨延长 ,于牵开结束时观察膝关节软骨的组织病理学变化。结果 A组 (延长 15 % )软骨细胞增多伴空泡变性。B组 (延长2 5 % )出现裂隙 ,表层和移行层中软骨细胞片状坏死。C组 (延长 35 % )关节软骨部分或全层缺损。基质沙黄O染色浓度A组轻度丧失 ,C组严重丧失。Mallory染色显示胶原纤维 ,A组排列规则 ;B组增粗 ,走行紊乱 ;C组破坏严重。结论 肢体延长中 ,关节软骨可能发生压力坏死 ,其发生率及程度与关节软骨所受压力大小有关。延长幅度越大 ,压力会越大。
