Objective:To induce the specimen of ischemic necrosis in adult rabbits femoral head and to develop a satisfied animal model for clinical and experimental study.Methods:34 adult China white rabbits were divided into 4 groups.In group A,10 rabbits were given endotoxin from Escherichia intravenously 2 times at an interval of 24 hours.In group B,10 rabbits were given endotoxin as in group A and were injected intramuscularly with methylprednisolone 3 times at intervals of 24 hours after the second injection of endotoxin.In group C,10 rabbits were given methylprednisolone intramuscularly 3 times at intervals of 24 hours.In group D,4 rabbits were used as the control.All of the rabbits were killed at 1 month after their final injection.Results:For light microscopic examination,the typical osteonecrosis were found in group B,the trabeculae or marrow necrosis were evident with apposition bone formation ,and there were also apparent thrombosis in the intraosseous microcirculation.For transmission electron microscopic examination,clear cell necrosis or cytolysis of bone were found in group B.Conclusion:Exogenous steroid appeared to potentiate the endotoxin reaction and the magnitude of hypercoagulability and osteonecrosis.It mainly increase the endothelial damage and hypercoagulability of those intraosseous and extraosseous vessels that subsequently thrombosed,cause the ischemic damage of femoral head.These results apparently indicated that,in the state of hypercoagulation,osteonecrosis was easy induced after steroid administration.

目的:诱导激素性股骨头缺血性坏死标本,为实验和临床研究提供满意的动物模型。方法:选中国大白兔34只,随机分为4组。A组:10只,静脉内注射大肠杆菌内毒素,共2次,间隔24h。B组:10只,注射内毒素同A组,在第2次注射完大肠杆菌内毒素后随即肌肉注射甲基强的松龙,共3次,间隔24h。C组:10只,单纯肌肉注射甲基强的松龙,共3次,间隔24h。D组:4只,对照组。将动物在最后一次注射后1mo处死,取股骨头标本,做组织学检查。结果:光学显微镜证实,在B组中,出现了较为典型的骨坏死,表现为骨小梁、骨髓坏死及髓腔内出血,骨内微循环有明显血栓或脂栓形成。透射电镜证实在B组中,骨细胞明显溶解坏死。结论:皮质类固醇激素能增加细菌内毒素反应导致明显高凝血状态及骨坏死。主要是造成了骨内外血管内皮损害,加重了内毒素引发的高凝血状态从而继发血栓形成,引起骨细胞的缺血性损害。表明在系统高凝血状态下应用激素,容易诱发股骨头缺血性坏死的产生

Objective: To establish an experimental model of femoral head necrosis with the administration of large dosages of dexamethasone (DEX) in rabbits and observe the pathological changes of the femoral head during and after DEX administration Methods: DEX of 2 5 mg·kg -1 ·day -1 was injected to the rabbits for 8 weeks and the histopathological changes of the femoral head of 36 rabbits were observed with light microscopy, transmission electron microscopy and radiography Results: ①In the 4th week after DEX treatment, typical necrotic foci appeared in the subchondral region of the femoral heads and the change became progressively severe along with the elapse of time In the 8th weeks, the rate of empty osteocytic lacunaes was (51 8±11 7)%and the marrow tissue was also extensively necrotic After the cessation of DEX administration, the necrotic changes were more aggravated In the 6th week after cessation, the rate of empty lacunaes was increased to (72 9±11 4)% Although the activity of bone repair was increased, new bone formation was less in the weight bearing region than in other regions ②Under electron microscopy, osteocytes were found to be degenerative and necrotic and disappeared eventually ③In the 6th to 8th weeks of DEX treatment, irregular radiolucent cystic areas in the subchondral region of the femoral head were found on the X ray films In the 6th week after DEX treatment, the radiolucent areas still existed and new bone was formed around them Conclusion: The administration of large dosages of dexamethasone can result in femoral head necrosis and the pathological changes continue on even the administration of dexamethasone is stopped This may be one of the important factors to cause collapse of the joint surface in the later phase of the healing of femoral head when the activity of bone repair is not sufficient in the weight bearing region of the femoral head Our study of dexamelhasone induced femoral head necrosis demonstrates the basic pathogenic features and the pathological development

目的 :采用大剂量糖皮质激素诱导实验性股骨头坏死 ,观察用糖皮质激素过程中和停药后股骨头的变化。方法 :给家兔注射 2 5mg·kg-1·d-1的地塞米松 ,光镜、透射电镜和X线摄片观察股骨头的病理变化过程。结果 :① 4周光镜下股骨头国软下区出现典型骨坏死灶 ,且随时间的延长而加重。 8周软骨下区平均骨陷窝空虚率达 ( 5 1 8± 11 7) %,骨髓坏死广泛。停药后骨坏死程度加重 ,6周骨陷窝空虚率为 ( 72 9±11 4) %;负重区新骨形成少。②电镜下见骨细胞经历了变性、坏死、溶解的病理过程。③ 6~ 8周X线片示股骨头出现不规则透亮区 ,停药 6周仍存在且伴周围骨质硬化。结论 :大剂量糖皮质激素可直接导致股骨头坏死 ,停药后骨坏死病程仍继续发展 ;负重区骨修复能力差可能是股骨头坏死塌陷的重要病理基础之一 ;地塞米松诱导的兔股骨头坏死反映了该病的基本病变特征和病理过程。

The cause of bone marrow mecrosis is manifold and mainly related with neoplastic diseases. It is seldom diagnosed before death and only can be found at autopsy. The main symptoms are bone pain and fever. Marrow aspiration and/or biopsy show necrotic sign and sometimes carcinoma cells can be found. An effective treatment for bone marrow necrosis is to treat the basic diseases.

引起骨髓坏死的原因是多种多样的 ,但最主要的原因是由恶性赘生性疾病引起 .该症诊断上有一定难度 ,常常尸解时才被发现 ;主要的症状有骨痛、发热 ;骨髓抽吸及活检有特征性改变 ,有时在骨髓检查中可发现癌细胞 .该症主要的治疗是对原发病的治疗 .
