Objective To evaluate the value of diffusion weighted imaging (DWI) in distinguishing between brain abscess and ncrotic and/or cystic brain tumor.Methods Twelve patients with brain abscesses (in which 10 cases were histologically verified with operation and 2 were clinically confirmed with follow-up), and 50 patients with necrotic and/or cystic brain tumors confirmed histologically (in which 28 cases were of astrocytoma, 10 were of metastasis, 6 were of hemangioblastoma and 6 were of acoustic neurinoma) were examined before operations or medical treats using conventional MRI and DWI.Results On DWI, brain abscess showed marked high signal intensity and necrotic and/or cystic brain tumor presented low signal intensity.Conclusion DWI has imoportant value in distinguishing between brain abscess and necrotic and/or cystic brain tumor. It should be used as a basic sequence as other conventional MRI sequences.

目的 评价磁共振弥散加权成像 (diffusiionwieghtedimaging ,DWI) ,在脑脓肿与坏死囊变性脑肿瘤鉴别诊断中的价值。方法  12例经手术病理 ( 10例 )或临床随访 ( 2例 )证实的脑脓肿和 5 0例经手术病理证实的坏死囊变性脑脓肿 (其中星形细胞瘤 2 8例、脑转移瘤 10例、血管母细胞瘤 6例、听神经瘤 6例 ) ,在术前或治疗前接受了常规MRI检查及DWI。结果 脑脓肿 (脓腔 )在DWI上表现为明显的高信号 ,而脑肿瘤坏死囊变区表现为低信号。结论 DWI对脑脓肿与坏死囊变性脑肿瘤的鉴别诊断具有重价值 ,应与常规MRI序列一起列为基本序列。

Objective To explore the value of amino acids determined at Multivoxel 2D (~1H )MR spectroscopy in differentiation between brain abscess and necrotic or cystic brain tumours. Methods Thirty-seven patients with histologically proved (12 patients with abscess and 25 patients with brain tumours) were examined before operations using conventional MRI and Multivoxel two dimensional (2D ) (~1H)-MRS, amino acids acids were analyzed in these patients. 目的探讨多体素2D1H-MRS的多种氨基酸峰测定在脑脓肿与坏死囊变性脑肿瘤鉴别诊断中的价值。方法对经手术病理证实的12例脑脓肿和25例坏死囊变性脑肿瘤,在术前行常规MRI检查及多体素2D1H-MRS分析,分析脑脓肿与坏死囊变性脑肿瘤病灶区的多种氨基酸峰出现情况。结果12例脑脓肿病例中10例病灶区可见分布各异的多种氨基酸峰包括0.9ppmAA峰(缬氨酸、亮氨酸、异亮氨酸)、Ala峰(苯丙氨酸)以及Gly峰(甘氨酸),而所有坏死囊变性脑肿瘤病灶区均未见上述代谢产物峰。结论多体素2D1H-MRS的多种氨基酸峰测定,在脑脓肿与坏死囊变性脑肿瘤鉴别诊断中具有重要价值。
