ObjectiveTo analyze the aberration changes of human eyes after LASIK through WASCA aberrometer.MethodsA prospective,randomized clinical study was taken in 86 eyes of 46 myopia patients received LASIK.Preoperative and postoperative examinations were performed.The patients were assigned to three different groups according to the diopter before the surgery.ResultsIn the examination of WASCA,C8,C9 and C12 were significantly changed following LASIK(P<0.01) among the high,moderate and low myopia groups.Preoperatively,there were no significant difference of coefficients,but after surgery,C12 showed great difference among the three groups(P<0.05).Significant differences of PV OPD,RMS PV OPD and PV OPD HO were revealed among three groups(P<0.01),but no for RMS PV OPD HO.In postoperation,RMS PV OPD,PV OPD and PV OPD HO were decreased,but RMS PV OPD HO was increased in comparison with that of preoperation.ConclusionLASIK can induce the change of ocular aberrations.The higher of myopia is,the larger of PV OPD.Postoperatively,high order aberration show difference without statistical significance in comparison with that of preoperation.

目的 通过WASCA波前像差分析仪检查和分析LASIK手术前后眼波前像差的改变。方法 对序列病例的 46人 86眼进行术前术后波前像差检查。分高、中、低 3个屈光度组。结果 各组内术后C8、C9、C12的改变有显著性差异 (P <0 0 1)。术前三组间的各项系数无显著性差异 ;而术后 10d及 1个月时 ,C12在三组间有显著性差异 (P <0 0 5 )。三组间术前的RMSPVOPD及PVOPD、PVOPDHO有显著性差异 (P <0 0 1) ,而RMSPVOPDHO无显著性差异。各组内术后的RMSPVOPD、PVOPD、PVOPDHO均比术前减小 ,有显著差异 ,但RMSPVOPDHO比术前增大。结论 屈光手术可以改变眼部的像差。屈光度愈高 ,总体像差愈大 ,术后变化愈大。但在不同的屈光度组间 ,高阶像差术前术后有差别 ,没有显著性。

Objective:To compare the results of spherical power(DS),cylinder power(DC),and astigmatic axis(AA) measured with lower order aberration to that of clinical refraction.Methods:Ninety six adults with myopia(192 eyes) were enrolled,39 males(78 eyes) and 57 females(114 eyes). Their ages ranged from 18 to 41 yr(25.89±5.95). The subjects were measured with an autorefractor,retinoscope and aberrometer under dilated pupil conditions. The results of DS,DC and AA were analyzed.Results:Four methods were used to measure the mean values of DS,DC and AA:cyloplegic autorefraction,(-5.11±2.16)D、(-0.61±0.16)D、86.60±73.85;cyloplegic retinoscopy,(-5.13±2.67)D、(-0.62±0.70)D、79.77±79.38;dry subjective refraction,(-5.30±2.15)D、(-0.64±0.73)D、81.67±75.22;and cyloplegic aberrometer,(-4.91±2.20)D、(-0.69±0.53)D、89.88±68.95. Under dilated pupil conditions,the DS from the aberration procedure was significantly lower than that for retinoscopy and autorefraction(P<0.05). The DS of retinoscopy was higher than for cycloplegic autorefraction. The DS under normal pupil conditions was higher than under dilated pupil conditions.Conclusion:There are differences between spherical refractive errors using an aberrometers than other refraction methods. This difference is related to pupil size,refractive error and the method of measurement. [

Objective To evaluate the pupil center shift quantitatively after pupil dilation and to investigate its influence on the outcome of wavefront guided LASIK. Methods Sixty-three eyes in 32 myopic patients with order RMS higher than 0.35 or were divided into groups A and B, and the Zyoptix LASIK was applied to the patients of both groups In group A, pupil center was used as the center of ablation, while in group B the ablation center was adjusted based on the pupil center shift. Results A (0.173±0.114)mm mean pupil center shift was observed in this study, with the maximum 0.51 mm. At 3 months after the operation, there was no significant difference in visual acuity and refraction power between the two groups. However, the reduction of coma was (0.025±0.137) μm and (0.097±0.118) μm in groups A and B respectively (t=2.131, P=0.037). The change in spherical aberration was significantly different between the two groups. Conclusions Pupil center shift after dilation is common. Adjustment of ablation center based on the shift may improve the correction of coma.

目的探讨根据散瞳后的瞳孔中心调整切削区对波前引导的个体化准分子激光原位角膜磨镶术(LASIK)疗效的影响。方法瞳孔直径为6mm时高阶像差均方根(RMS)>035μm的近视患者32例(63只眼),随机分为A、B两组,接受Zyoptix个体化LASIK手术。A组以瞳孔中心为切削区中心,B组则根据测得散瞳后瞳孔中心的偏移量对切削区中心作相应调整。结果散瞳后瞳孔中心平均发生(0173±0114)mm的偏移,最大达051mm。术后3个月两组患者视力、屈光度数差异无统计学意义(P>005),波前像差检查显示手术前后彗差的差值分别为(0025±0137)和(0097±0118)μm(t=2131,P=0037),而球差差异无统计学意义(P>005)。B组彗差的矫正效果优于A组。结论散瞳后瞳孔中心普遍发生偏移,根据其偏移量在术中对切削区中心作相应调整可提高彗差的矫正效果。(中华眼科杂志,2005,412426)
