Objective To observe and evaluate the practical value of anterior interior fixation with Z-plate on the treatment of spinal thoracolumbar fracture. Methods 8 patients with thoracolumbar fracture and spinal cord injury were received anterior decompression, bone graft and Z-plated interior fixation. Results All patients were followed up for 6-18 months after operation and have a relative stable fixation. Conclusion Z-plate system can be advocated for clinical application.

目的观察并评估Z -plate钢板在胸、腰段骨折前路内固定术中的作用和价值。方法对 8例胸、腰椎骨折并脊髓损伤病人行前路减压植骨 ,采用Z -plate钢板作内固定。结果术后随访 6~ 18个月 ,固定牢靠。结论Z -plate系统在材料、设计、专用工具匹配等方面具有较高的科学性、先进性 ,值得推广。

(2)Z-plate system is made of titanium alloy; it has good rigidity,strength,and histocompatibility, hardly interferes with MRI. It is an ideal fixer for rebuilding stabilization of spines by anterior approach.

目的 在前路重建胸腰段椎体稳定应用Z-plate系统的体会。方法 总结前路应用Z-plate系统治疗5例患者及重建脊柱稳定的疗效。经5~8月的随访,均获骨性融合,脊柱稳定性得以重建。3例神经损伤中(以ASIA分级)术后有2例改善2级以上,1例改善1级。无钢板和螺钉松动或脱落。结论 (1)Z-plate系统具有固定可靠,骨性愈合好,较传统的前路钉棒系统使用简单。(2)Z-plate系统是钛合金,其刚度、强度和组织相容性好,对MRI检查干扰小,是理想的前路脊柱稳定性重建内固定器械。

目的 :对三种胸腰椎前路内固定器进行生物力学评价。方法 :采用新鲜成人胸腰椎标本 (T12 ~S1) 15具 ,制成前、中柱损伤模型 ,分别以DunnⅢ、NTP、Z plate固定 ,进行强度、刚度和稳定性的研究。结果 :胸腰椎骨折采用三种前路内固定器固定后 ,在脊柱的强度、刚度和稳定性方面 ,NTP、Z plate均优于DunnⅢ (P <0 0 5 ) ,而NTP和Z plate无显著性差别 (P >0 0 5 )。结论 :NTP和Z plate系统符合脊柱的生物力学 ,能有效恢复失稳脊柱的稳定性 ,是较为理想的前路内固定器。
