The subset of lymphocytes in normal human blood, the immunologic changes in AIDS patients' blood and the immunophenotype of bone marrow cells in acute leukemia were analyzed using flow cytometry and monoclonal antibodies reacted with lymphocyte and myelomonocyte. The percentages of CD2, CD3, CD4, CD8 CD19, CD20, CD21 cells and the absolute numbers of CD2, CD3, CD4, CD8 cells were generated. The ratio of CD4/CD8 was 1.83±0.36 (n=40). There was a dramatic decrease in CD4 cells in AIDS: The percentage from 47% droped down to 15-25%, the absolute number from 784 down to 175-344/mm~3, the ratio of CD4/CD8 was 0.68±0.24 (n=6, P<0.01). There was a significant decrease compared with the ratio of normal control. CD4 cell level is an important indicator for diagnosis and prognosis of AIDS. ALL was differentiated to T-ALL defined by T11, Leu9, Leul and non-T-ALL defined by Ia, B4, CALLA, B1, Cμ and SmIgG. Ia was present in all cases of non-T-ALL. B4 and B1 were the model antibodies for B cell lineage. The cells with Cμ were considered less mature than the cells with SmIgG. Myeloid surface markers My4, My7, My9, Mol and Mo2 were used for discriminating between myeloid and lymphoid leukemia. 17 cases of AML were all expressed Myg. My4 and Mo2 were also present in 6 cases who were considered as monocytic leukemia. Two-color flow cytometric analysis has shown that the hairy cells possessed with a concomitant reactivities with the B cell antibody Leu14 and the monocytic antibody Mo5. This character can be used as a diagnostic indicator for hairy cell leukemia. In our study, the flow cytometry was used for clinical diagnosis and immunosurveillance. The standardization of flow cytometric methodology and the instrument calibration were discussed.
