Objective To explore the T staging, value and limitation of MRI in esophageal carcinoma.Materials and Methods The barium meal film and MRI scan data of 31 cases with esophageal carcinoma proved pathologically were analyzed. Among them, lower esophageal and cardia were both involved in 7 cases. 4 cases were also performed CT scan at the same time, 3 cases of postoperative specimen were carried out MRI scan.Results According to the T staging of MRI in 25 cases of postoperative esophageal carcinoma, there were T_ 1-2 stage (n=7), T_3 stage (n=13), T_4 stage (n=5), lymph nodes metastasis (n=6) and liver metastasis (n=2). To compare MRI scan data with the pathologic findings, the whole accuracy of the T staging of MRI in 25 cases was 80.0%, and 71.4% in T_ 1~2 stage, 76.9% in T_3 stage, 100% in T_4 stage respectively. Kappa value=0.678,P<0.0005,which illustrated the high concordance.Conclusion MRI is an effective supplement of the barium meal examination, which is useful in preoperative T staging and clinical treatment.

目的探讨MRI对食管癌检查的可行性、T分期及其价值和限度。资料与方法搜集31例病理证实,行消化道造影和MRI检查的食管癌患者资料,其中7例腹段食管癌累及贲门。4例同时行CT检查,3例术后标本行MRI检查。结果25例行开胸手术的食管癌患者,MRIT分期结果包括T1~2期7例,T3期13例,T4期5例。淋巴结转移6例,肝脏转移2例。与病理学结果T分期结果比较,MRI对食管癌T分期判断的整体准确度为80%(20/25),其中T1~2期为71.4%(5/7),T3期为76.9%(10/13),T4期为100%(5/5)。Kappa值=0.678>0.4,近似概率P<0.0005,说明两种检查的一致程度较好。结论在食管癌的影像学检查中,MRI是对消化道钡剂造影有效的补充检查方法,主要用于术前分期,可为临床治疗方案的制定提供重要的依据。
