Afterthe operation,irritating cough,barking, swallowingand free movement of neck didn'tcause istula.Naked-eyes andtissue examination showed that thehealing ofthe anastomostic orifice was well and no stenosis wasfound. The glue membrane was absorbed completelywithin 1~2months. Outside body anti-pulling forceexperiment (for 8 cases ) showed that the anastomosticline can hold 100cm H_2O stress and 2kg pullingstrength if only OB glue was used to connect the re-sected ends. Hence , this experiment shows that theusage of OB glue for tracheal anastomosis is a safty,simple and convenient method

作者将活体犬的颈段气管,按实验要求切断,将两端用丝线缝合3针,然后在吻合口上涂OB胶液,在其固化成胶膜前,用疏松之气管鞘包绕。术后未引起吻合口瘘,病理组织学检查证实愈合良好。离体犬气管切断后,用胶粘接(不加缝合),经抗压力及抗拉力实验的结果表明,其吻合口可承受9.8067kPa压力和2.0kg拉力而不裂开。作者认为,用OB胶粘接气管吻合口,是一种安全可靠、简单省时的气管吻合方法。

胃内灌注药物及胃肠道营养是目前临床工作中的重要治疗措施 ,广泛应用于内、外、妇、儿各临床科室 ,为挽救患者生命及提高患者生存质量起了很重要的作用。目前胃内灌注药物不足之处是药物的吸收不佳 ,副作用大 ,胃肠道营养不充分吸收 ,肠道手术后的吻合口瘘发生率存在。我们研制的双腔十二指肠气囊管的双腔及气囊 ,可有效延长药物吸收时间 ,减少药物副作用 ,延长营养吸收时间 ,控制营养物质摄取量。经床6 3例试用 ,安全可靠 ,效果良好 ,并可大大降低吻合口瘘的发生。该项目已获国家专利 ,专利号 992 476 19 4。

Objective: Attempts have been made to replace esophageal defects with artificial prosthesis composed of biodegradable and nonbiodegradable materials, which is gradually replaced by host tissue. Methods: The artificial esophagus was a two layered tube consisting of a collagen sponge matrix and an inner polyurethane stent. A 5 cm esophageal segmental defect was replaced using the artifial esophagus in twenty dogs. Nutritional support was given after operation. The inner polyurethane stent was removed endoscopically...

Objective: Attempts have been made to replace esophageal defects with artificial prosthesis composed of biodegradable and nonbiodegradable materials, which is gradually replaced by host tissue. Methods: The artificial esophagus was a two layered tube consisting of a collagen sponge matrix and an inner polyurethane stent. A 5 cm esophageal segmental defect was replaced using the artifial esophagus in twenty dogs. Nutritional support was given after operation. The inner polyurethane stent was removed endoscopically after 4 weeks fasting. Results: Four dogs (20%) died from anastomotic leakage. Fourteen dogs (70%) survived more than 1 month, and only two (10%) suffered from esophageal stricture. Three of the fourteen dogs survived for 8 months up to now without any complications. Complete epithelization of the regenerated esophagus was observed 1 month after surgery. Complete regeneration of esophageal mucosa structures was observed 3 months after surgery, and partial regeneration of esophageal muscle tissue was observed 6 months after surgery. Conclusion: The feasibility that is the artificial prosthesis of polyurethane collagen in reconstruction of the cervical esophagus segment of the dogs is warranted. Esophageal high order structures can be regenerated providing a temporary support by polyurethane stent and an adequate three dimensional structure for a sufficient period by collagen sponge.

目的 采用生物可降解材料与非降解材料复合制成人工食管 ,通过诱导自身食管组织爬行再生与生物材料降解相匹配 ,最终新生食管完全替代人工食管。方法 将医用聚氨酯内管表面覆盖海绵状胶原蛋白膜制成人工食管 ,并通过手术置换 2 0条犬颈段 5cm食管缺损 ,术后给予营养支持 ,禁食4周后通过内镜取出聚氨酯内管。结果 术后发生吻合口瘘 4条 (2 0 % ) ,14条 (70 % )存活 1个月以上 ,其中食管狭窄 5条 (2 5 % ) ,存活达 8个月的 3条 ,现仍存活良好 ,未见明显并发症。术后 1个月 ,新生食管完全上皮化 ;术后 3个月 ,粘膜全层结构完整再生 ;术后 6个月 ,食管肌层部分再生。结论 聚氨酯 -胶原蛋白复合人工食管置换犬颈段食管具有可行性 ,通过聚氨酯内管提供暂时支撑 ,海绵状胶原蛋白膜提供适宜细胞爬行再生的三维支架并维持足够长的降解时间 ,能够促使自身食管的完全再生。
