Objective: To introduce the method and mew technique of auricle reconstruction in congenital microtia. In the first stage we employed the caved cartilage framework combining the instantly expanded skin and utilized ear tissue to reconstruct the lobule, trague and concha cavity. Objective: To introduce the method and mew technique of auricle reconstruction in congenital microtia. In the first stage we employed the caved cartilage framework combining the instantly expanded skin and utilized ear tissue to reconstruct the lobule, trague and concha cavity. In a second stage we set up the cartilage framework to finish the structural support for a new auricle. The wound of 22 patients were all well healed primarily after that stronger structural appearance of reconstructive auricle can be viewed beneath the skin in the region of an absent auricle. During the first stage we completely used micro remnant and ear tissue, design flaps and cartilage flaps to reform the lobule, trague and concha cavity. All patients got satisfactory appearances of new ears. The key to the technique is how the three-dimensional cartilage framework be sculptured for a new auricle Instantly expanded skin in the ear region as well as fully utilize of micro remnant and ear tissue improving the contour of reconstructive auricle all are helpful to create an excellent auricle.

报道22例先天性小耳症耳廓再造的方法及改进技术。采用软骨支架立体雕塑方法结合耳后支架埋置处皮肤急性扩张,及利用小耳组织重建耳垂、耳屏及耳甲腔,Ⅱ期手术耳廓掀起成形,行耳廓再造22例。结果22例患者,Ⅰ期耳廓支架埋入后均完全愈合,Ⅰ期术后可见埋置于皮下的立体感较强的再造耳形态,术中充分利用小耳组织,设计皮瓣及软骨皮瓣,Ⅰ期重建耳垂、耳屏及耳甲腔,取得良好效果,Ⅱ期耳廓掀起成形,外形满意。在耳廓再造术中,耳廓软骨支架的立体雕塑是本技术的关键,耳后皮肤的术中急性扩张,及充分利用小耳组织,改善再造耳廓形态,有助于塑造更完美的耳廓

Objective:To explore the use of different skin flaps in reduction of the partial absence of auricle.Methods:Fourteen cases were treated with different flaps,including retroauricular pedicle flap,retroauricular musculocutaneous flap,chondric flap in medial auricle combined with retroauricular flap,and preauricular flap with retroauricular flap.All the cases were followed up.Results:Among them,12 cases got satisfied effect,the other 2 were delayed in healing because of the uncomplete debridgement and overusing...

目的 :探讨耳郭部分缺损手术修复的方法及不同皮瓣的临床应用。方法 :对耳郭部分缺损的 14例采用耳后带蒂皮瓣 ,耳后肌为蒂的皮瓣 ,耳郭内侧软骨皮瓣、耳后皮瓣联合整复和耳前、耳后皮瓣联合整复等 4种不同的手术方法和皮瓣进行一期整复 ,并术后随访。结果 :14例中除 2例因清创不彻底和电刀使用过重致皮瓣远端延迟愈合外 ,余均一期愈合 ,整形效果满意。结论 :设计的 4种不同皮瓣及手术方法均能一期整复耳郭部分缺损 ,适应证广 ,成活率高 ,值得临床推广使用
